    COMPLAINT-AFFIDAVIT

    I, _____________________, of legal age, Filipino, with address at


    _____________________, under oath, depose and state that:

    1. I am a voter of ____________________, under Precinct No. ________.


    2. On May 9, 2022, while I was on my way to the precinct to cast my
    vote, I saw Mr. B, Ms. S, and Mr. X just outside the school and heard
    them talking about the ongoing elections.
    3. I was just minding my own business when I heard Mr. X said, “Sige
    ha, mupusta ko ug five hundred, si ABC lagi na ang daug.” Ms. S
    agreed but Mr. B replied, “Ay, sure ko JKL lagi na, five hundred sad
    ako.” Finally, they agreed that whoever will win will take the money.
    4. I hurriedly approached my lawyer and then I was told that one of the
    Prohibited Acts under Sec. 261 of the Omnibus Election Code is

    c. Wagering upon result of election. - Any person who bets or


    wagers upon the outcome of, or any contingency connected
    with an election. Any money or thing of value or deposit of
    money or thing of value situated anywhere in the Philippines
    put as such bet or wager shall be forfeited to the government.

    charges for Violation of Sec. 261 (c) of the Omnibus Election Code
    against the said Respondent, who may be served with subpoena and
    other processes of this Honorable Office at (his/her) last known address
