    ALL DOCUMENTS SUBMITTED

    I, JESSAMAE BADAR MACASOJOT, of legal age, single, Filipino and a resident of Brgy.
    105, Suhi District St. Francis Village, Tacloban City, after having been duly sworn to an oath in
    accordance with law, hereby depose and say;

    I am in possession of the original copies of the following documents, to wit:

    1) CSC Form 212 (Revised 2017) in two copies with the latest 2x2 ID picture;
    2) Certified photocopy of PRC Professional Identification Card;
    3) Certified photocopy of LET Rating
    4) Certified copy of Transcript of Records (TOR) with General Weighted
    Average (GWA) signed by University Registrar
    5) Certified copies of Service Record and Performance Rating
    6) Certified copy of Certificate/s of Specialize Training
    7) Certified copy of voter`s ID and/or any proof of residency as deemed
    Acceptable by the School Screening Committee
    8) Certified copy of NBI Clearance

    I have caused the faithful reproductions from the original documents as being required relevant
    to my application to enter into a government service as teacher.

    I am executing this affidavit to attest to the truth of the foregoing facts and for whatever legal
    purpose this may serve.
