INTRODUCTION:-

As an increasing proportion of population and with the shift in disease patterns


from acute illnesses to chronic illnesses, the traditional disease management and
care focus of the health care professions has expanded. The health care focus
must center more on prevention, health promotion and management of chronic
conditions than in previous times.

Ambulation Meaning:-it the ability to walk from place to place with or without assistance.
In ambulation individuals able to walk at least 10 feet outside the parallel bars with supervision or
physical assistance from only one person.

DEFINITION:-
Ambulatory care is a personal health care consultation ,treatment or intervention using advanced
medical technology or procedure delivered on an outpatient basis.(I.e where the patients stay at the
hospital or clinic,from the time of registration to discharge.)
CLASSIFICATION OF AMBULATION

1. Ambulatory-dependent-dependent for physical assistance.


Patient requires manual contacts of no more than one person during
ambulation.manual contacts are continuous and support body weight.

2. Ambulatory-dependent for physical assistance.


Patient requires manual contacts of no more than one person during
ambulation.manual contact consist of continues or intermittent light touch for
assistance.
3. Ambulatory-dependent for supervision
Patient can physically ambulate on level surface without manual contact with
another person but for safety requires stand by guarding.
4. Ambulatory independent level
Patient can ambulate independently on level surface but requires supervision.
5. Ambulatory-independent
Patient can ambulate independently on non level and level surface,stairs and
inclines
AMBULATORY CARE SETTINGS

 Hospital emergency department

An emergency department (ED), also known as an accident & emergency


department (A&E), emergency room (ER), emergency ward (EW) or casualty department, is a
medical treatment facility specializing in emergency medicine, the acute care of patients who
present without prior appointment; either by their own means or by that of an ambulance. The
emergency department is usually found in a hospital or other primary care center.
What are the critical condition-

 Community outreach programme


Community Health Outreach Program is a team of health professionals including registered
nurses and community health advocates. ... Emphasis is placed on health promotion, prevention,
and developing links to community resources through health screenings, education and referrals .
 Medical clinics
  Medical clinic means an organization of medical professionals providing physical or mental health service or
surgical care for the sick or injured.

 Ambulatory care center.


The new Ambulatory Care Center (ACC) – part of Hamad Medical Corporation (HMC) – focuses on
delivering outpatient and day surgery services in Hamad Bin Khalifa Medical City

Ambulatory care includes surgical day care and outpatient clinical services and Hamad Medical Corporation
(HMC) is among the growing number of healthcare providers designing and building facilities to cater for short term
surgical stays of less than 24 hours.

The facility is equipped with 66 beds – 12 pre-operation bed spaces and 54 post-anesthesia care unit
(PACU) recovery beds. In addition there is 14 operating theaters. 

The 33,400-square-meter ACC offers urology, podiatry, gastroenterology, otolaryngology, audiology and
ophthalmology services. It also houses a pre-assessment clinic where patients undergo a thorough medical
evaluation to ensure readiness for surgery. 

The patient care process is designed to produce the best possible clinical outcome, maximize access and
convenience for our patients as well as offer as many treatments and procedures as possible on an ambulatory
basis.

 Cardiac rehabilitation canters.


Cardiac rehab is a medically supervised program designed to improve your cardiovascular health if
you have experienced heart attack, heart failure, and angioplasty or heart surgery. Cardiac rehab has
three equally important parts:

 Exercise counseling and training: Exercise gets your heart pumping and your entire
cardiovascular system working. You’ll learn how to get your body moving in ways that promote
heart health.
 Education for heart-healthy living: A key element of cardiac rehab is educating yourself: How
can you manage your risk factors? Quit smoking? Make heart-healthy nutrition choices?
 Counseling to reduce stress: Stress hurts your heart. This part of cardiac rehab helps you identify
and tackle everyday sources of stress.

AMBULATORY CARE NURSING


Ambulatory care includes those clinical, management, educational and research
activities provided by registered nurse and with individuals who seeks care and
assistance with health maintenance and or health promotion.(AAACN).

ROLE OF NURSE IN AMBULATORY CARE

 Patient care:-providing direct patient care.

 Screening:-conducting patient intake screening.


 Treating:-treating the pt with acute or chronic illnesses or emergency condition.

 Referring:-referring patient to other agencies for additional services.

 Teaching:-teaching the patients about self care activities.

 Offering health education programme that promote health maintenance.

 Clinical manager:-nurses are also work as a clinical manager, direct the operation of
clinics and supervise other health team members.

 Emotional support:-Providing advice and emotional support to patient and family


members.

 Record:-patient’s medical histories and symptoms help the patient for follow-up and
rehabilitation.

 Counselling:-counselling to the family members of critically ill patients.

 Warnings sign and symptoms of disease:-registered nurses work to promote
general health by educating the public on various sign and symptoms of disease
and where to go for help.

 Registered nurse also might run general health screening or immunization clinics,
public seminar on various conditions.

 Some ambulatory care nurses are involved in tele health, providing care and advice
through electronic communication media such as video conferencing and internet.

CONCLUSION:-

Using this systematic approach, hospitals and health systems can quickly plan, identify
and execute on a successful ambulatory strategy to better organize and coordinate
care for their patient populations. This framework provides a clear model for organizing
future ambulatory services to best serve the patients in their specific communities.
