Preface

Situated in the Southwestern corner of New Jersey bordering the Delaware River, Salem
County is an unspoiled oasis with a “down-home country” feel that is rich in a variety of
agricultural, historical, cultural, and economic features. In close proximity to major cities such as
Philadelphia, New York City, Baltimore and even Washington D.C., Salem County remains
connected economically and industrially while also retaining distinct small-town communities
filled with family-owned businesses that keep this County feeling like home. Due, in part, to a
large network with its neighboring communities and cities, the population of Salem County
boasts a diverse reputation consisting of people from many different ethnic and national
backgrounds.
To ensure that the needs of the total population are met, Salem County offers a wide
assortment of social support services and programs that are sure to serve each individual person’s
unique situation. Services that distinctly guarantee that our aging population is taken care of are
offered from several different agencies across the County while programs to serve the migrant
farm worker and Hispanic populations continue to assist with building and supporting solid
networks and ensuring their continued access to every resource and opportunity that Salem
County has to offer. In addition, Salem County perpetually strives to overcome the social
inequities of housing insecurity, food insecurity, and lack of substantial mental health and
substance abuse resources that certain populations in this country face by supporting and
promoting services to address these challenges superficially and long-term.
The goals and missions of each agency outlined in this document are one in the same: to
serve and promote the health and wellbeing of all persons, regardless of status, that live, work,
and educate in Salem County. As a whole, it is our hope that this manual will ensure that the
population of Salem County continuously and consistently has access to a robust compilation of
information on the social support services and programs that are offered here.

Cleaning Services
Housekeeping Service (PRAC):

This service provides light or heavy housecleaning services such as laundry, dusting, mending,
cleaning, meal preparation, washing floors, and more by a non-professional staff for those Salem
County residents 60 years and older. Performance of routine errands, such as grocery shopping
or purchasing prescribed medications. To inquire about this service, contact Geovanni Cantizano

Communications
Dial My Calls- Salem County Office on Aging and Disabilities

This is an automated calling system used to provide information on upcoming events, activities,
medication reminders, and holiday notices of the Salem County Department of Health and
Human Services. This service is primarily targeted to seniors (60+), persons with disabilities, or
their caregivers. To be added to the call list, contact the Salem County Office on Aging and

Ready Salem- Salem County Office of Emergency Management

Ready Salem is an alert program in Salem County that allows individuals to sign up to receive
information about emergencies and other important community news. The system allows county
officials to provide enrolled participants with critical information quickly in a wide variety of
situations such as severe weather, missing persons, and evacuations of buildings or
neighborhoods. Interested individuals may add home, mobile, work, and/or business numbers in
addition to email addresses, text message numbers, and more. For more information or to register,

Salem County Office on Aging & Disabilities Senior and Disabled E-News

E-News is an email program to connect individuals with upcoming events or new programs in
the office. To participate and receive updated information, please contact the Salem County

Salem County Department of Health & Human Services LINCS Notification System

The Salem County Department of Health Public Preparedness Division oversees and manages a
Local Information Network Communication System (LINCS) for disseminating public health
information from the New Jersey State Department of Health and Center for Disease Control and
Prevention (CDC). This system is designed for individuals to enroll and receive up to date public
health news and information regarding public health incidents that occur in Salem County. For

Disability Services
The ARC of Salem County

The ARC of Salem County serves and supports individuals with intellectual and developmental
disabilities in Salem County. The ARC helps people to navigate the process of qualifying and
keeping services for individuals with disabilities such as adults with developmental disabilities
who need a day program to assist with reaching their fullest potential, formerly employed adults
who need vocational evaluation, school aged individuals transitioning from school to work,
families needing respite time from caring for a family member with a disability, people with
significant mental illness interested in employment, and many more. For more information,

Register Ready

Register Ready is a confidential statewide online database that enables individuals with special
needs to self-identify so that in the event of an emergency, first responders are aware of their
needs, where they reside, and any other additional information pertinent to their condition. To

Resources for Independent Living

Resources for Independent Living (RIL) is an organization that provides services to individuals
with severe disabilities, including populations deemed unserved or underserved. The core
services provided include independent living skills, information and referral, transition, peer
support, and advocacy. Membership is open to all people with disabilities, their able-bodied
friends, and supporters. In addition, RIL promotes personal growth and provides people with
disabilities the means for self-choice, self-direction, and self-reliance in an inclusive society that

Head Start

Head Start is a federal program that promotes the school readiness of children ages birth to five
from low-income families by enhancing their cognitive, social, and emotional development. For

Rutgers Southern Jersey Regional Child Care Resource and Referral Agency

Rutgers Southern Jersey Regional Child Care Resource and Referral Agency is funded by the NJ
Department of Human Services/Division of Family Development. The mission of Rutgers
Southern Regional CCR&R, a non-profit organization operated under Rutgers University
Camden, is to promote and support access to affordable, quality early childcare and education for
young children in southern New Jersey by offering financial resources and education to parents
and professional development and technical assistance to the early childhood community. For

Equipment Assistance
Loan Closet

Loan Closet is a program designed to allow individuals to borrow gently used items such as
walkers, wheelchairs, and canes. If you are interested or in need, please contact Salem County

Exercise Programs
MOVE Yourself/Walk N Talks- Salem County Office on Aging and Disabilities

MOVE Yourself/Walk N Talks is a program offered by the Salem County Office on Aging and
Disabilities that allows Salem County residents 60 years and older to attend exercise classes
twice per week throughout the year to promote health and wellness. Various exercise tapes are

played throughout the program and health education information is provided after the workout as

Project Healthy Bones

Project Healthy Bones is a 1-1.5 hour, 24-week once a week exercise and education program for
older adults at risk or who want help to prevent osteoporosis. The curriculum includes exercises
that target the body’s larger muscle groups and are designed to improve strength, balance, and
flexibility. It also educates participants on the importance of exercise, nutrition, safety, drug
therapy, and lifestyle factors as they relate to osteoporosis. For more information on the schedule

Tai Chi for Arthritis:

Tai Chi for Arthritis is an evidence-based program created in 1997 that consists of classes to
target movements and positions that are beneficial for individuals with arthritis. For more

Family Services & Support


The Families First Discovery Pass Program

The Families First Discovery Pass program provides families and individuals who receive
benefits through SNAP, WFNJ, the Child Care Subsidy Program and/or WIC free or steeply
discounted admission to museums, theaters, historic sites, cultural events and more across the
state. Discounts are available by showing your NJ Families First Card or your WIC Identification

Family Success Centers

Family Success Centers are “one-stop” shops that provide wrap-around resources and supports
for families before they find themselves in crisis. Family Success Centers offer primary child
abuse prevention services to families and bring together concerned community residents, leaders,
and community agencies to address the problems that threaten the safety and stability of families
and the community. There is no cost to access services provided by Family Success Centers
(FSCs). There are three Family Success Centers in Salem County. Please refer below for those

The Huddle of South Jersey

The Huddle is a nonprofit organization that works with fathers to teach them parenting and life
skills as well as provide information and education on a variety of practical life topics. For more

Youth Advocates

Youth Advocates is a national nonprofit organization committed to keeping youth and adults
‘safely home’ with their families, supported by their community. YAP provides young people,
adults, and their families with extensive support right from where they live, work, learn, and play.
In addition, YAP is committed to bringing racial equity to youth justice, child welfare and
numerous other system partners where disparities have persisted for decades. For more

Financial Assistance
Federal Emergency Paid Sick Leave

receive federal tax credits for providing certain employees up two weeks of paid leave to
workers who become sick with Covid-19, or up to 12 weeks of family leave to people

Federal Family and Medical Leave Act

 Description: Federal program to provide an employee or certain members of an
employee's family who are sick up to 12-weeks unpaid, job protected leave in a
designated one-year period. Allows employees to retain group health insurance.
 Eligibility: NJ employees who are unable to work due to illness or caring for certain
family members who are ill. Speak with HR of employer for specific qualifications and
application process.

MIPPA (Medicare Improvements for Patients and Providers Act):

MIPPA is a program for seniors and disabled persons on Medicare who may qualify for extra
help. To be eligible for this program, your resources must be limited to $12,890 for an individual
and $25,720 for a married couple living together. Your annual income must be limited to
$18,732 for an individual or $25,368 for a married couple living together. Vehicles and houses
do not count as resources when calculating your total resource value. To initiate the screening

NJ Earned Sick Leave

 Description: NJ Law in which most employers are required to provide up to 40 hours of
paid sick leave to employees. This sick leave may be used by employee to quarantine
based on advice of healthcare provided or public health authority, to get tested for
COVID-19, take care of children when school or childcare closed due to an epidemic
care for themselves or loved one with COVID-19, or attend school related meeting,
conference, or event. An employer's policy must meet or exceed NJ's sick leave law. For

 Eligibility: NJ employers who work full-time or part-time. An employer is not required to
pay sick leave to the following employees: individuals employed in the construction
industry under a union contract, per diem health care employees, public employees who
are provided with paid sick leave under any NJ law or rule, and independent contractors
who do not meet the definition of employee under NJ Law.

NJ Family Leave Insurance

Pandemic Unemployment Assistance

 Description: Federal Program expanding eligibility to receive Unemployment benefits up
to 85% of a recipient’s weekly wage up to $881 a week.

COVID-19. Participant must have earned at least $200 per week during 20 or more
weeks in the base year period or must have earned at least $10,000 in total employment
during the base year period.

Worker’s Compensation

 Description: Program is a "no fault" insurance program providing medical benefits,
temporary total benefits, permanent partial benefits, or permanent total benefits to
employees who suffer job-related illness or injury. More information can be found at

 Eligibility: Workers injured on the job. NJ law (SB 2380) creates a rebuttable
presumption of workers' compensation coverage for COVID-19 cases contracted by
"essential employees" during a public health emergency. (Seek counsel from a WC
attorney to discuss further.)

Congregate Nutrition

Salem County has one center where a nutritionally balanced meal is served. Recreational
activities are also provided at the center. New applicants must call Tamu Lewis at Mid-Atlantic
States Career and Education Center at 856-514-2200 ext. 107 to determine eligibility. The site is
open from 9a-1p.

Disciples Pantry

Farm Market Vouchers- Salem County Office on Aging and Disabilities

The Salem County Office on Aging and Disabilities provides free farm market vouchers to
individuals who live in Salem County, are 60 years or older, and have an income that does not
exceed $23,606 if single or $31,894 if married. Individuals who wish to apply for farm market
vouchers must call the office at 856-339-8622 to obtain an application. Vouchers are given out at
various locations in Salem County to individuals who bring their completed application along

First Assembly of God Hands of Love Food Pantry

First Wednesday Luncheon

Each month on the first Wednesday of the month a free luncheon and program is offered to
Salem County residents 60 years and older or disabled residents under 60. The purpose of the

Childhood Lead Program

The Salem County Department of Health and Human Services provides the Childhood Lead
Program for children who may have been exposed to high levels of lead in the home. The
Nursing and Environmental Divisions will coordinate with the child and his/her guardian to
ensure routine blood testing to monitor the child’s blood lead level as well as inspect the home to
ensure the lead is removed by a certified contractor. In addition, educational materials are always

Matter of Balance is a program designed to reduce the fear of falling and increase activity levels

Project Healthy Bones

Project Healthy Bones is a 1-1.5 hour, 24-week once a week exercise and education program for

flexibility. It also educates participants on the importance of exercise, nutrition, safety, drug

The Right to Know program is offered by the Salem County Department of Health and Human

Caregiver Support Program

The Caregiver Support Program provides assistance to caregivers providing care for someone 60

Jersey Assistance for Community Caregiving (JACC)

JACC is a long-term care program that provides a broad array of in-home services and supports
to anyone 60 years or older in need of nursing home level care who needs to remain in his/her
homes. A uniquely designed package of supports is intended to supplement and strengthen the
capacity of caregivers and to delay placement in a nursing facility. JACC is not a Medicaid
program and requires cost share based on income. The individual may own a house and a car and
have countable resources below $40,000 for an individual or $60,000 for a couple. For further

MLTSS (Medicaid Managed Long Term Services and Support):

Medicaid Managed Long Term Services and Supports (MLTSS) is a Medicaid waiver program
that is designed to provide expanded community-based care for eligible participants. Services
may include home delivered meals, home care, and PERS in addition to the Medicaid healthcare
services that a participant is eligible to receive. Participants will work with a care manager from
his/her Managed Care Provider (MCO) to develop a plan of care that best suits his/her needs. To
be considered eligible, individuals must be 65 years or older, eligible for Social Security
Disability, or declared disabled by the State of New Jersey. An individual’s income for 2021
must be below $2382/month with assets below $2,000. For more information and to initiate the

PASP

The Personal Assistance Services Program (PASP) is a self-directed program that provides
routine, non-medical assistance to adults with disabilities who are attending school, employed or
involved in community volunteer work. Personal assistants help with tasks such as meal

preparation, light housekeeping, bathing, dressing, shopping, and driving or using public
transportation. The total number of hours of assistance a person can receive per week is variable
but cannot exceed 40 hours per week. To be eligible, you must reside in New Jersey, have a
permanent physical disability, be between 18-70 years of age, be capable of directing your own

Project LifeSaver- Sheriff’s Office

Project LifeSaver is a program that provides enhanced support and assistance to Salem County
residents who are primary caregivers for individuals with Alzheimer’s disease or a related
dementia disorder who are at risk of becoming lost or wandering away from home. In addition,
Project Lifesaver is also a program and resource for individuals with cognitive and
developmental disabilities. When enrolled in this program, participants wear a special arm
bracelet that has the ability to assist local law enforcement in locating the individual by using a

Southern Jersey Family Medical Centers Dental Clinic

Southern Jersey Family Medical Centers provides dental care for children and adults at

Smile for a Lifetime

Smile for a Lifetime is an international program that provides orthodontic care to disadvantaged
children and young adults who normally would not be able to afford treatment. Smile for a
Lifetime does not charge for the orthodontic treatment – participating orthodontists and
orthodontic manufacturing companies provide FREE services, including the cost of one set of

Hearing Aid Assistance to the Aged and Disabled (HAAAD)

HAAAD is a program provided to elderly individuals or those with disabilities that reimburses
partial amounts to offset the cost of hearing aids. HAAAD will provide a $500 reimbursement or
up to $1000 reimbursement for two hearing aids per calendar year. One reimbursement is
allowed per calendar year. Individuals are eligible for this program if they are enrolled in the
Pharmaceutical Assistance to the Aged and Disabled (PAAD) program or Lifeline Utility
Assistance program and meet those specific criteria and if they make less than $28,769 if single
or less than $35,270 if married. Individuals who are enrolled in the Senior Gold Prescription
Discount Program and are a Medicaid recipient who receives full hearing aid coverage are NOT

Reconditioned Hearing Aid Project

The New Jersey Hearing Aid Project provides reconditioned hearing aids to low-income seniors.

Sexual Assault Response Team/Sexual Assault Nurse Examiner (SART/SANE)

SART/SANE is a program through the state of New Jersey that provides services to victims of
sexual assault, namely healthcare treatment. Under SART/SANE, victims of sexual assault are
provided thorough care in well equipped, specialized facilities that allow for collaboration
between law enforcement, medical professionals, and rape care advocates. In accordance with
the Attorney General’s Directive, the SART/SANE activation occurs when the following criteria
are met:
 The victim must be 13 years of age or older
 The incident must have occurred within 5 days of disclosure
 The victim must consent to the activation of SART.

PAAD- Pharmaceutical Assistance to the Aged and Disabled

PAAD is a program available to senior citizens 65 years and older or persons with a disability
receiving Social Security Disability. The yearly income of individuals must be below $28,399
for a single person and under $34,817 for a couple. This program enables eligible individuals to
purchase prescription medications for $5.00 for generics and $7.00 for brand name. Individuals

Potassium Iodide (KI) Pills- Salem County Department of Health Preparedness Division

The Salem County Department of Health and Human Services Public Preparedness Division has

Salem/Hope Creek Nuclear Generating Station. Municipalities that fall within this radius are

Property Tax Reimbursement

The Property Tax Reimbursement Program provides those who are 65 or older collecting Social
Security Disability reimbursement for increased property taxes paid on the home in which they
own and reside. There are income eligibility limits each year. Applications are usually available
in February by calling 1-800-882-6597.

Senior Citizen and Disabled Persons Property Tax Deduction


 Provides a $250 annual deduction on property tax bill.
 Must renew annually.
 Eligibility
o 65 years or older or a person with a disability (with a Physician’s certificate or

CERT/MRC/DASH, or Community Emergency Response Team/Medical Reserve


Corps/Domestic Animal Sheltering Team, is a combined volunteer group comprised of 3
volunteer groups that work very closely in emergency management and preparedness situations
and events. This group is run by the Salem County Department of Health and Human Services
Public Preparedness Division and the Salem County Office of Emergency Management.
Volunteers in CERT/MRC/DASH assist with yearly free flu clinics held around Salem County,
natural disasters, emergency incidents, and public health activities. In addition, monthly trainings
are held where volunteers are exposed to a wide variety of situations and information to ensure

Child Find is a free referral service and public awareness campaign to assist in the identification
of unserved/underserved youth with a delay or disability from birth through twenty-one years of

Childhood Lead Program

The Salem County Department of Health and Human Services provides the Childhood Lead
Program for children who may have been exposed to high levels of lead in the home. The
Nursing and Environmental Divisions will coordinate with the child and his/her guardian to
ensure routine blood testing to monitor the child’s blood lead level as well as inspect the home to
ensure the lead is removed by a certified contractor. In addition, educational materials are always

Children’s Specialized Hospital

Children’s Specialized Hospital offers free early developmental screening. Schedule your child’s
developmental screening through video or phone call from the comfort of your own home. The
30-minute virtual screening includes an interactive questionnaire for parents and children. Each
family receives personalized feedback, recommendations, and education materials. Screenings

Early Intervention

Children develop at different paces, but there are developmental milestones that children should
reach by certain times during the first three years of their lives. When children fail to reach these
milestones, parents and other caregivers should consider contacting Early Intervention for an
evaluation. The cost of Early Intervention services for a child is determined by their family
income. Families whose income is less than three times the federal poverty level will receive
Early Intervention Services at no cost. If you suspect that an infant or toddler may be

PerformCare

PerformCare is the Contracted System Administrator (CSA) for the New Jersey Children's
System of Care (CSOC), serving as the single point of entry for all children, adolescents, and
