Dear Physiotherapist,
I am writing to request a home visit for Mr. Stephen Kerry who suffered a right knee injury. He
requires further assessments and management for him to regain his mobility.
A week ago, he fell down the stairs due to extreme headache which resulted to his current injury. He
was referred by his GP to We Care Home Nursing Agency for support with dressing, assisting in
taking shower daily and pain management as needed.
At his recent visit to the agency, warm compress was applied initially, and proper wound dressing
was done. He complained of usual pain while walking in a little distance, but no infection noted.
After some time, he was able to walk using walking sticks for support.
He had a cerebrovascular accident acquired 2 years ago. He was alert, mentally active, speech
slightly slurred by then but walked with limp and with impaired balance.
I hope that you could visit him soon to start the therapy so that he can do his usual routine. Kindly
discuss with Mrs Jennifer Kerry, his wife, how regularly physiotherapy will be needed, and the
exercises best for him.
