LEARNING CONTRACT
I am taking part in this learning contract because I am officially enrolled in NSG 124.6 Section E as
part of the curriculum of Bachelor of Arts in Nursing. This contract is in effect until the end of 2nd
semester, A.Y. 2022. I am expected to comply with the following actions and strategies listed here
which will facilitate my knowledge and help me perform well in the course:
Attendance: I will attend the scheduled synchronous sessions set by the faculty member.
This is either live video conferencing or live conferencing in an online platform. If I will not
be able to do so, I will duly inform my instructor and choose other options to catch up with
the course discussions and activities by watching the recorded sessions and the uploaded
videos. Attendance is being recorded and counted by attending the scheduled activities and
commenting on the designated thread/comment section for learning reflection and
interactive class discussion.
Simulation Videos/Skills Testing Videos. Being a Laboratory and Skills Subject,
improvement in skills is one of the important criteria for learning. As adjustment to
the online learning, I will be submitting video showing simulations of a scenario or
performing a certain procedure. The Videos must show the specific steps of the procedure. I
am aware that the skills testing videos will be assessed by my instructor and feedback
will be sent back to me.
Learning Activity Outputs. For every procedure in this course, I must answer the checklist
by providing the purpose, considerations, safety measures related to procedure,
and
rationale for each step. Filled-out checklist will be submitted back to the instructor in a clear
image format. I am aware that the clinical instructor, upon her own discretion, might
provide additional self-structured learning activities that will help me in improving my
knowledge and skill on the subject matter.
Study/Assignment/Requirements: I will allocate at least 3 hours every week to spend on
the course. I will submit all the output expected of me on its due time. I am aware that any
delay in the submission of course requirements will affect the submission of my
grade. Hence, I will fully comply with the final course requirements preferably a week before
the deadline of grade submission to allow ample time for checking and grade verification
and prevent any problems and delays in enrolment.
Examination: I will answer exams to the best of my knowledge and will full honesty and
integrity.
Course Help: I will download and read the course guide to be fully informed about the
course requirements and grading system. I will communicate with due respect and courtesy
with our instructor through the designated FB group and email-add for
classroom, questions and clarifications.
SKILLS TESTING/RETURN DEMONSTRATION GUIDELINES. I am fully aware of the
following specific guidelines in skills testing and demonstration:
1. Return demonstration of procedures can be done at home for video recording or for live virtual
assessment as guided by the following:
a. Procedure must be personally done by the student, and clearly shows the student’s face during
the performance.
b. Proper blocking is necessary to ensure that the skill and steps can be properly checked
for accuracy and efficiency.
c. Start the video return demonstration by giving brief definition and purpose of the
procedure. Stating the rationales while performing the skills is highly recommended.
d. Steps of the procedure can be done either verbatim (word-for-word) from the checklist or
by using your own words, provided that these steps are precisely done according to the checklist.
e. The equipment can be resourced from the available materials that you have at home and or can be
created inventively.
f. Proper decorum must be exercised throughout the entire video presentation (e.g. wearing
of complete uniform, use of appropriate language etc.)
g. The use of loud background music or sound effects is not allowed. Please opt for instrumental or
soft music if you wish to incorporate sounds on your videos.
2. Recorded videos of return demonstration can be sent online (via Telegram app,
Google Classroom or other easy file transfer programs/applications) or Via USB to those who have
poor network connectivity. Please observe social distancing and wearing of complete PPEs.
(please do notify your clinical Instructor ahead of time if you wish to submit face-to-face thru flash
drive.)
3. Pursuant to the mandate of the University to observe maximum leniency during this pandemic,
the student will be given 2-week allowance from the set deadline of submission of return
demonstration to complete due requirements. Should the student fail to submit output after the 2-
week allowance, zero grade for that specific activity/requirement will be given and
submissions made after the given allowance will no longer be accepted.
