I Ankush dey, resident of Powai station, do hereby, declare the following that
1. I have read the Instructions, Guidelines and relevant orders of Govt. of India pertaining to COVID-19 pandemic
2. I have read the Information bulletin related to this examination available on the website : https://ao.nta.ac.in
3. I have, in the last 14 days (please tick ✔, wherever it is applicable to you, otherwise leave blank ):
a) the following u-like symptoms:
Fever , Cough , Breathlessness , Sore throat / Runny nose , Body ache

b) been in close contact with a con rmed case of COVID-19 (“Close contact” means being at less than one metre distance for more than 15 minutes.)
c) not been in close contact with a person su ering from Covid-19 and am NOT under mandatory quarantine:
d) Travelled to the following cities/ countries in the last 14 days prior to arriving at the Centre.
4. The health and wellbeing of our community is our rst priority; therefore the Centre reserves the right to deny entry to its premises.
5. I have read the detailed “IMPORTANT INSTRUCTIONS for CANDIDATES” and “ADVISORY for CANDIDATES REGARDING COVID-19”, and I undertake to abide by
the same.
The above undertaking has to be lled up in advance before reaching the Centre, except candidate's signature which has to be done in the presence of
the invigilator. No candidate will be allowed to enter the test Centre, after the gate closing time mentioned on the admit card.
IMPORTANT INSTRUCTIONS FOR CANDIDATES
1. This Admit Card consists of four pages – Page 1 contains the Centre details and Self Declaration (Undertaking) form regarding COVID-19; Page 2 and 3
contain "Important instructions for candidates" and Page 4 contain Advisory for candidates regarding COVID-19". The candidate has to download, read and
understand all the pages. However only the top sheet (Page 1) containing the personal and test details as well as the Self Declaration (Undertaking) and
signatures of parent/ candidate, is required to be submitted to the invigilator at the exam centre
2. All candidates are required to download and read carefully the Instructions and Advisory for COVID-19 given with the Admit Card and strictly adhere to
them.
3. As soon as Admit Card is downloaded by the candidates, they have to check the details carefully and bring discrepancies, if any, to the notice of NTA
immediately by way of a mail at [email protected] stating their Application Number, Roll Number and mobile phone number or contact the NTA Help Desk at
011-4075 9000 between 10.00 am and 5.00 pm.
4. The Admit Card is provisional, subject to satisfying the eligibility conditions as given in the Information Bulletin.
5. The Admit Card is not transferrable to any other person. Impersonation is a legally punishable o ence.
6. Request for change of examination centre will not be entertained. If a candidate appears at an examination centre other than the one indicated in his/her
Admit Card, his/her candidature will be cancelled.
7. It is suggested that the candidates may verify the location of examination venue, a day in advance so that they do not face any problem on the day of
examination, in locating the centre.
8. As a precaution for COVID-19, the candidate must reach Centre as indicated against Reporting/Entry time at Centre in the Admit Card.
9. No candidate shall be permitted to enter the Centre after the Gate Closing Time, i.e after 8:00 AM (For Morning session ) & 01:00 PM (For afternoon
session)
10. No candidate would be allowed to enter the Examination Centre, without Admit Card (only the top sheet containing the personal and test details as well as
the Self Declaration (Undertaking) and signatures of parent/ candidate), and Valid ID Proof.
11. Candidates are permitted to carry only the following items with them into the examination centre:
Ball point Pen (Blue/Black)
Admit Card along with Self Declaration (Undertaking) downloaded from the NTA website (a clear printout on A4 size paper) duly lled in.
Personal transparent water bottle
Personal hand sanitizer (50 ml)
Mask and gloves
Original and valid ID proof.(PAN card/School ID Card (with Photo)/Passport/ Aaadhar Card(With photograph)/E-Aadhaar/Ration Card/Aadhaar
Enrolment No. with Photo. All other IDs/ Photocopies of IDs even attested/scanned photographs of IDs in mobile phone will NOT be considered as valid
ID Proof. In case the candidate does not have any of the abovementioned ID proof, he/she should carry a letter from his/her School Principal/Head
Master, containing his/her details such as Name, Father's name, Name of the school and Class in which the candidate is studying, with his/her
photograph and signature duly attested, by the Principal/Head Master
Medical certi cate as applicable in the case of PwD candidate.
12. Before reaching the Centre, the candidates must enter required details in the Self Declaration (Undertaking) in legible handwriting, a x the Photograph,
a x Left-Hand Thumb Impression and Candidate’s Parent’s signature at the designated places. They should ensure that their Left-Hand Thumb Impression
is clear and not smudged. The candidate has to a x his/her signature, in the presence of the Invigilator in the examination Room/ Hall, on the day of
examination
13. Candidates are NOT allowed to carry any personal belongings to the Examination Centre as there will be no safe keeping arrangements at the centre and
Examination O cials will not be responsible for safekeeping of personal belongings
14. The following items are prohibited at the exam premises: - (a) Mobile phones or any other communication device including Bluetooth etc (b) Textual or
stationery material such as a book, paper, formulae, log tables, whitener, slide rule (except blue or black ball point pen) (c) Personal items such as wallet,
purse, cap, goggles, jacket, food, drinks, pointed instruments etc. Candidates must check and ensure that they do not have any of the barred/ prohibited
items. If they realize that they are carrying any such item, it must be handed over immediately to the invigilator. Any infringement of these instructions will
invite disciplinary action and withdrawal of candidature. If any other candidate(s) is using unfair means, please object and report immediately to the
invigilator.
15. Candidates will be required to occupy only the seats allotted to them and are not allowed to change their seat in the examination hall.
16. Candidates are advised to listen to the instructions of the Invigilators at the Examination Centre before commencement of the test.
17. The question booklet will be supplied 10(Ten) minutes before the commencement of the examination.
18. While opening the transparent cover of the sealed test booklet/ OMR answer sheet, candidates are required to exercise caution not to damage the
contents.
19. There will be 4 sets of question booklet viz. 'A', 'B', 'C' & 'D'. Candidates are required to check carefully that the question booklet number is printed on the
cover page and the question booklet contains the same number of pages as mentioned on the cover page. Candidates should also satisfy themselves that
the language of the question booklet is the same as mentioned on the admit card.
20. In case of any discrepancy in the question booklet, candidates should report the matter to the Invigilator immediately.
21. Candidates are required to mark their answers on the OMR answer sheet, contained within the question booklet. They are not allowed to open the seal of
the question booklet until they are advised to do so by the invigilator. The OMR answer sheet is to be handled carefully without folding or dis guring it.
Only the relevant elds and circles are to be lled in the OMR answer sheet and no other scribbling or rough work should be carried out on the front or
reverse of the OMR. Candidates may use the question booklet to do the rough work.
22. Candidates are required to ll up their particulars on the OMR Answer Sheet. They should note that any omission/mistake/discrepancy in lling up details
on the OMR Answer Sheet, especially with regard to Roll Number, Question Booklet, OMR responses, etc. will render the answer sheet liable for rejection.
Hence candidate should not rush to ll the details and do it carefully. In case the candidate commits an error while lling the OMR answer sheet, the sheet
will not be replaced. The onus of lling the correct information in relevant elds of the OMR Answer Sheet is solely on the candidates who may consult the
Invigilator in case of doubt.
23. Candidates should darken/ bubble/ shade the circle which represents, in their opinion, the correct answer, completely, so that the letter/ number inside
the circle is not visible. Darkening of multiple circles against a question number will render the answer invalid and no marks will be given for such multiple
answers. The correct and wrong methods of darkening the circle on the OMR answer sheet are given below:-
NTA will implement Social Distancing measures as per Government of India guidelines in current scenario of COVID-19 to ensure health & safety of the
candidates. Adequate measures are implemented for safety of all without compromising the high standards, sanctity and fairness in conduct of the
examination. Candidates are also required to adhere to the guidelines and for Social Distancing and hygiene to ensure safety & health of their own and fellow
candidates. For safety purposes, NTA strongly advises candidates not to bring anything other than permitted items.
Preparation at Centre
1. Standard Operating Procedures for implementing safety precautions and for maintaining required standard of hygiene are getting implemented.
2. Seating area will be thoroughly sanitized. All door handles, staircase railing, lift buttons, etc will be disinfected.
3. Hand Sanitizer will be available at the entry and inside the exam venue at various places for candidates and centres to use
4. The Room/ Hall number where they will be writing the Exam will be informed to the candidate inside the Registration Room.
5. It is ensured that all the processes are touch free to ensure Social Distancing norms.
Pre-Exam Preparation
1. Candidate to check Reporting/ Entry time at Centre given in the Admit Card and to reach centre as per Reporting time only, to avoid any crowding at the
time of entry and to maintain social distancing
2. Candidate should ll Admit Card and Undertaking completely and properly, as per the instructions
At time of Entry
1. Candidates need to maintain a space of at least 6 feet from each other at all times. Floor Marks will be arranged outside the centre. Follow the instructions
provided by Centre Sta .
2. Room/ Hall number will not be displayed outside the centre to avoid any crowding at any place in any situation
3. Candidates will be required to sanitize hands by washing with soap and with Hand Sanitizer before entry in centre. Hand sanitizer will be available at
various locations in the centre.
4. Candidate should bring duly lled Admit Card and Undertaking as per instructions.
5. Candidates would display their documents for veri cation from a safe distance.
6. Do not bring prohibited items to Exam Centre. Please refer to Point-14 of the IMPORTANT INSTRUCTIONS FOR CANDIDATES given on Page-2 for permitted
items
7. At the time of entry, the lled in Undertaking on Admit Card will be checked and centre sta will guide you to respective room/hall. Candidates are
required to strictly adhere to instructions provided by centre sta .
8. Please observe that no one will be denied permission to appear in the examination, unless he/she violates the COVID19 directives/advisories of
Government (Central/State) applicable on the day of exam and instructions mentioned in the Admit Card.
During Examination
1. Candidates can sanitize their hands with sanitizer available in the examination room/hall.
2. Candidates are required to sign on the Attendance Sheet and Admit Card after sanitizing hands with sanitizer.
3. IIn order to stop chances of any unfair means being used in the examination, the candidate is expected to wear the freshly provided mask at the centre in
the examination room. The candidate may keep the mask worn by him/ her outside the exam hall with other possessions. Used masks or gloves should be
disposed only after reaching home and NOT thrown anywhere inside the exam centre premises or on the way home.
After Examination
1. On completion of exam, the candidates will be permitted to move out in an orderly manner one candidate at a time. Candidates are required to wait for
instructions from the Invigilator. They should not get up from their respective seats until advised.
2. Duly lled Admit Card (only the top sheet containing the personal and test details as well as the Self Declaration (Undertaking) and signatures of the
parent/ candidate, must be handed over to the Invigilator at the end of the examination. Failure to do so may result in non evaluation of the OMR answers
sheet.
