    AFFIDAVIT OF UNDERTAKING/
    RECOGNIZANCE

    I, ROMMEL MIRANDA, of legal age, Filipino, married and a


    resident of Tagumpay 1, Brgy. Caminawit, San Jose, Occidental
    Mindoro, after having been sworn to in accordance with law, do
    hereby depose and say:

    1. That, I am Barangay Kagawad of Caminawit where Reyvie


    Bautista is residing and who is currently detained at Occidental
    Mindoro Provincial Jail in Brgy. Magbay, San Jose, Occidental
    Mindoro for the crime of Violation of Sec. 5, Article II, R.A. No.
    9165 before the Regional Trial Court of San Jose, Occidental
    Mindoro, Branch 45;

    2. That, I undertake to assume full responsibility and duty to


    supervise and guide the Accused during her release on
    recognizance pending approval of his Motion for Probation
    before this Honorable Court and the Probation Center,
    pursuant to our laws and regulations;

    3. That, I likewise solemnly promise and undertake before this


    Honorable Court to produce the body of the Accused whenever
    required to do so during the pendency of the above-cited cases
    in order to answer the charges;

    In view thereof, I have hereunto affixed my signature on this 6 th


    day of October, 2021, at San Jose, Occidental Mindoro.
