Accident Involved With Derrick Crane

On March 2009, an incident which


involved a derrick crane occurred at
construction site. The incident caused
immediate death of two workers at the
scene after crashed by a bucket filled
with bricks. The incident occurred during
lifting of a bucket fully loaded with bricks
from ground floor to the 34th floor.
During lifting of the bucket, suddenly the
wire rope snapped. It caused the bucket
to fall and crashed onto workers who
were standing at the loading area. There
were two signalman during incident who
were at ground floor and level 34. The
derrick crane was installed at top level of
the construction building which at level
39 and is used to dismantle tower crane.

Recommendations:

1 Access to construction site must be safe and labelled


. properly.

2 Hoisting wire rope for lifting must in good condition at


. every time derrick crane is installed and operation.

3 Ensure effective communication between signalman,


. loading workers and crane operator.

4 Practice safe lifting by prohibiting anyone from walking


. under the load during lifting activity.

5 Any lifting activity must be supervised by Safety and


. Health Officer even during weekends.
Fire And Explosion In LPG Storage

On April 2009 approximately 11.00 pm, there was a fire and explosion in storage
of liquefied petroleum gas (LPG). The storage was double storey shop houses.
The accident caused one fatality, one suffering from serious burn and minor
injuries to four others. The vibration of the explosion was felt 3 km radius away
and also caused serious properties damage. During accident, it was estimated
about 400 cylinders with capacity 14 kg and 15 kg kept in the store. Two lorries
full with LPG parked in the premise were destroyed.

LPG is classified as flammable substances and is colourless, odourless and


heavier than air. Presence of LPG can be detected by the distinctive pungent
odour due to injection of ethyl mercaptan as an odorant. The LPG releases cloud
flash and explode if it comes in contact with ignition source. LPG cylinder may
experience BLEVE (Boiling Liquid Expanding Vapour Explosion) if exposed to
open flame.

Recommendations

1 The workers should been trained to detect any leaks from the cylinders by
. these following methods:

a. presence of the distinctive pungent odour due to injection of ethyl


mercaptan as odorant used in LPG; or

b. slightly larger leaks will generate a "hissing" sound or frosting around


leaks.

2 Storage arrangement should ensure the following:


.

a. storage area must have adequate ventilation;


b. full cylinders should be stored separately from empties, thus making an
easy demarcation of LPG existence area;

c. cylinder should be located away from any heat/source of ignition; and

d. storage area must have good housekeeping and adequate warning


signs and fire fighting equipment.

3 Information pertaining to hazard of LPG, safe handling procedure and what


. to do during emergency should be established, disseminated and
implemented.
Escalator Accident Involving

On January 31, 2010 and


approximately at 11:39 am a child
aged around two to three years of his
left leg was trapped between the
escalator skirt and the side of
escalator step while going down the
escalator. This accident resulted in
serious injuries to his leg where some
parts of his legs were broken.
Escalator is a type of transportation
that transports passengers from one
floor to another floor in a building such
as in the shopping complex. It consists
of stairs electric motor driven that
moves up or down a ladder platform,
while the step remained flat.

Here are suggestions to ensure


passenger safety while riding an
escalator: 
Make sure the shoes laces are tight
and shoes used are appropriate
when taking escalator;
Always hold children's hands on
escalators and do not allow children
to sit or play on the escalator step
when taking escalator;
Always look ahead, stand in the
middle of the steps where the
escalator step yellow colored
border, and hold on handrail;
Avoid standing on the side of the
step to avoid trapped leg;
Know where the emergency button
shutoff in red if you need to stop
the escalator;
Avoid placing bags or packages in
the handrail;
Do not lean on the side handrail.
