    Instructions:

    1. This grading sheet must contain the names of all the students in your class as entered in the control sheet
    arranged alphabetically.
    2. Computerize “INC” under “Remarks” if a students failed to earn a grade due to unfulfilled requirements or
    failure to take the examination due to valid reason/s and provided his academic performance during the
    Midterm Examination meets the passing mark.
    3. Use RED INK for grades of 5.0.
    4. Computerize “OD” or “UD” for students who Officially Dropped or Unofficially Dropped, respectively.
    5. Grading sheets should be printed in a long bond paper, with 11 as font size and Arial as font style. It must be
    “CERTIFIED CORRECT” by the College Dean.
    6. Print “NOTHING FOLLOWS” after the last entry.
