2021 Recommended Immunizations from Birth through 6 Years Old: www.cdc.gov/vaccines/parents/downloads/parent-ver-sch-0-6yrs.pdf. Based on this schedule, a nonmedical exemption
would be submitted at 2 months, 4 months, 6 months, 12 months and 18 months of age.
