Case Scenario

Case Scenario 10 Gestational Diabetes Mellitus

A 36-year-old pregnant woman in her 28th week of gestation was in her second pregnancy and has had one
live birth and no abortions is seen for prenatal care. Her weight is 220 lbs and her blood pressure is 120/80 mmHg.
The uterine size is appropriate for gestational age. The patient’s past obstetric history includes C-section with a 9 lb,
8oz baby girl at 39 weeks of gestation.

Upon interview, she reported that her mother and father are both with Type II Diabetes Mellitus diagnosed at
their 30’s and 40’s respectively.

A urine dipstick shows 3+ glycosuria and negative ketones. The patient's capillary glucose reading, performed
in the clinic, was 189 mg/dl. She was instructed to return the next morning for a fasting venous plasma glucose,
which was 151 mg/dl. A 50-g oral glucose tolerance test (OGTT) was administered and revealed a blood glucose level
of 155 mg/dl 1 hour after (a positive test is a venous plasma glucose value 1 hour later of > 140 mg/dl).

The patient was initially prescribed Glyburide 2.5 mg tablet OD. She was instructed to monitor her sugar
levels at home through a glucometer in the morning before taking anything per orem. Also, she was instructed to limit
intake of sweets and carbohydrates that would include a diet of 25 kcal/kg actual body weight divided into three
meals and a bedtime snack. The targets for therapy were a fasting value of < 95 mg/dl and values no higher than 140
mg/dl at 1 hour or no higher than 120 mg/dl at 2 hours after eating.

After 3 weeks of diet and drug therapy, her blood glucose levels remain elevated. Thus, medication was
shifted to NPH (Neutral Protamine Hagedorn) Insulin 20 units and regular insulin 10 units administered in the
morning.
