INFORMATION INCLUDING THE PROCESSING OF ANY SPECIAL CATEGORY OF PERSONAL INFORMATION AND SENSITIVE
PERSONAL INFORMATION
IMPORTANT NOTICE: WE ACKNOWLEDGE RECEIPT OF YOUR APPLICATION.
HOWEVER, BEFORE WE CAN CONSIDER IT FURTHER, WE REQUIRE YOUR SIGNED CONSENT IN RELATION TO ANY PROCESSING WE
MAY CARRYOUT IN RELATION TO YOUR PERSONAL INFORMATION under Regulation (EU) 2016/679 of the European Parliament
and of the Council of 27 April 2016 the General Data Protection Regulation (GDPR) on the protection of natural persons with
regard to the processing of personal data and on the free movement of such data and the Philippine Data Privacy Act of 2012
(DPA), as may be applicable.

Dear Sirs,
In order to progress my application for employment as a seafarer (as defined by Article II, paragraph 1 (f), MLC, 2006) by the
Marlow Navigation group of companies (further referred to as “the Company”), I hereby give my explicit consent for the Company
to collect, hold, process, store, transfer, and use my personal information, including any special category of personal information
and sensitive personal information as relevant, required and appropriate to my request for employment, which may be processed
both by way of computer media and on paper.
I understand that in order to review / process my request for employment and in order to comply with any mandatory
requirements of any law, my personal details may be provided to ship‐owners, relevant or issuing authorities, and other third
parties where there is a legitimate reason to do so. Such information shall include, but is not limited to:
1. Personal details and identification documents (name, address, contact details, birthdate, birthplace, age, gender, civil
status, religion, citizenship, signature, ID photo),
2. Professional qualifications / trainings (educational background, skills),
3. Medical certificates, medical records, medical test results, and medical history (only to the extent necessary for
screening and selection and enhancing health care),
4. Sea service, previous experience and recruitment information (positions held, dates of previous employment,
interviews and test results), and
5. Emergency contact (name, address, contact details).
As regards the medical certificates, medical records, medical test results, and medical history, I understand that the Company
may collect the same directly from me or from the clinics or the medical facilities that conducted and prepared such documents.
I further understand that such documents shall be processed for the purposes of screening and selection—to determine my
fitness for work—and for enhancing healthcare. I likewise understand that such documents may be disclosed to the Company’s
clients and/or the appropriate authorities, with such disclosure hinged on the Company’s legitimate interests and compliance
with applicable laws, rules and regulations.
Furthermore, I also understand that the Company is subject to the existing and future government regulations. I therefore agree
to be bound by all applicable domestic and international laws in relation to the processing of my personal data. In connection
thereto, I hereby give my consent to the Company to disclose and process the personal data provided, which would be accessed
by its authorized personnel for the following purposes (but not limited to) (a) screening and selection, (b) to provide information
to the Company’s principal and training providers in order to transact and/or process applicable documentation and/or items
required prior training, (c) for recording, reporting, historical and statistical purposes, and (d) to comply with any other
requirement that may be required by pertinent laws.
I acknowledge and agree that the Company may disclose my personal data to third parties, including, but not limited to (a)
regulatory bodies/agencies and other legal bodies, (b) banks and/or payroll processing service providers, (c) information
technology service providers, (d) partner medical clinics and other third parties and medical facilities handling medical screening,
(e) affiliated training centers, (f) external auditors and external counsel, (g) company global affiliates, and (h) suppliers.
It is my responsibility to provide correct and updated information to the Company in relation to my application. I accept that
should I be offered employment by the Company, the Company will then keep a record of any of my personal information
including any special category of personal information and sensitive personal information necessary for the performance of or in
relation to my employment contract. I understand that until the Company has received a notification of any changes to any of my
personal information, the Company shall act on the basis that all my personal information that the Company hold are in order,
complete and correct.
I acknowledge that I have a legal right under the relevant data protection laws to access any personal information on me that is
kept by the Company and that I also have the right to rectify such information where I can provide evidence that it is incorrect. I
also acknowledge my entitlement at any time to suspend or withdraw my consent to any further processing, and to order the
blocking, removal or destruction of my personal information held by the Company in accordance with my rights under the law. I
also acknowledge my right to be informed of the existence of the processing of my personal information and of any automated
decision‐making or profiling, as well as my right to be indemnified for damages sustained due to inaccurate, incomplete, outdated,
false, unlawfully obtained or unauthorized use of personal information, taking into account any violation of my rights and
freedoms as a data subject, and to lodge a complaint with the appropriate data protection authorities.
I understand that all personal data on me will, generally, be kept for a period of 10 years from the date of inactivity, and only for
as long as necessary. Should my application be rejected for any reason, I acknowledge and agree that personal data will be kept
for a period of 5 years.
I understand further that all personal data will be stored and processed in a manner that ensures appropriate security against
unauthorized or unlawful processing, accidental loss, destruction or damage using appropriate technical or organizational
measures.
I hereby confirm that I have read and understood, and agree to the processing of my personal or any special category of personal
information or sensitive personal information for reasons as outlined above for the performance of my Seafarer Employment
Agreement and notwithstanding that my explicit consent may not be required by virtue of Article 6 (a) to (e) and Articles 9 and
10 of the GDPR. or Sections 12 and 13 of the DPA.
