OBJECTIVE: To study working principle of Defibrillator machine.

WORKING PRINCIPLE: A defibrillator is a device used to treat ventricular arrhythmias.


Defibrillators have various components, including a power source, variable transformer, rectifier,
capacitor, switches and paddles. A rectifier, in the capacitor charging circuit, converts AC voltage into
DC voltage. DC energy is used rather than AC because it is more effective, causes less myocardial
damage and is less arrhythmogenic. A capacitor is the most important part of the defibrillator. A
capacitor is formed by a pair of conductors (metal plates) separated by an insulator (a layer of air). A
capacitor stores electrical energy in the form of electrical charge. In the capacitor, the quantity of
electrical charge stored for a given charge potential is determined by the surface area of the plates, the
thickness of the insulating layer and the ability of the capacitor to store charge (permitivity). During
the discharge of a capacitor, delivered energy falls exponentially and some of the energy available is
lost in circuit resistance, inductor and paddles. The defibrillator has an inductor in its output circuit.
The inductor gives optimum shape and duration to the delivered current. In a biphasic (newer type)
defibrillator, the effective energy required for defibrillation is less, requiring smaller capacitors.

DEFIBRILLATOR ANALYZER:

NETECH MODEL DELTA 3000:


Delta 3000 Defibrilator Analyzer combines the complete testing functions of a Defibrillator Tester,
Transcutaneous Pacemaker Analyzer, Transvenous Pacemaker Analyzer and 12-Lead ECG /
Arrhythmia Simulator into one compact, lightweight, and easy to-use instrument. No other single
instrument matches the breadth of functions or delivers the performance testing power of the DELTA
3000. Delta 3000 tests all Defibrillators including AEDs. It measures both monophasic and bi-phasic
waveforms. It tests all external Pacemakers including transthoracic, transvenous, and AV sequential
models. With 12 built-in user selectable test loads for transthoracic pacing testing, the built-in ECG /
Arrhythmia Simulator allows performance testing of any ECG equipment. Menu driven and easy to
use, the Delta 3000 displays test measurements on the large four line LCD. A print key outputs the
data to the RS-232 serial port. Included with the Delta 3000 is a hard carrying case, various paddle
adapters and interface cables.

PRE-OPERATION

REQUIREMENTS:
1. Manual External defibrillator with AED
feature
2. Netech Model Delta 3000 Defibrillator Analyzer.

OPERATION:
1. Switch on defibrillator and set on the initial value of energy.
2. Switch on Netech model delta 3000 defibrillator analyzer.
3. Take out the paddles of defibrillator and charge the paddles by pressing button on one of the
paddle or by pressing charge button on the control menu.

4. Make sure you are taken paddles in right position, apex paddle in right hand and
sternum paddle in left hand.
5. Discharge the defibrillator by placing the paddles on analyzer and press the shock buttons of
both paddles on the same time.
6. Note the values of charge delivered by defibrillator.
7. Deduce the impulse time by voltage and current values.
8. Repeat the procedure with five different values of energy set and observe the results.

POST-OPERATION:

PRECAUTION:

1. Clean paddles thoroughly


2. Place paddles accordingly in the socket.
3. Switch off DC power cable etc

healthcare providers in hospital and clinic settings. The LIFEPAK 20 defibrillator/monitor offers the

The following paragraphs provide additional information about the controls shown in Area 3

Contrast (Passive Display Only) Press the CONTRAST button and rotate the Speed Dial to adjust
the screen contrast/brightness. When the defibrillator is turned on, the contrast setting defaults to the
previously adjusted setting.

Home Screen The home screen is the background screen that displays during ECG monitoring.
Pressing HOME SCREEN returns you to the home screen from any menu screen or overlay, except
during AED analysis or during manual defibrillation charging and shocking.
Event After pressing EVENT, the screen displays the following overlay
Generic is automatically selected when EVENT is pressed and no other selection is made. The
selected event and time stamp appear in the message/status area on the screen.

You are on the duty in the emergency department when a 6kg 6 month old infant is
rushed in, limp and minimally responsive. Mother states that the baby has increased
respiratory efforts and has been feeding poorly mother thought he had a viral
respiratory infection.as you approach, the infant stops breathing and becomes
unresponsive.

CASE-2:An inconsolable baby!


You are called to help stabilize an 8.5kg, 6-month old female in the emergency department. The
baby’s mother says that baby was well until 2 days ago when he develops inconsolable fussiness and
poor feeding. The baby sometimes seems dusky during these episodes. The baby was afebrile, dusky,
and inconsolably crying on arrival. The cardiology fellow assigned to the emergency department
administered medication for what he thought was an abnormal heart rhythm. The infant did not
respond to the medication and the blood pressure has fallen. Cardiology now feels that a defibrillator
is needed and has asked you to bring it to the bedside.

CASE-3: A lethargic teen comes to the ED!


A 65kg, 17-year male is brought to the emergency department with history of lethargy and suspended
multidrug ingestion. He had been at a save and his friend called EMS when he seemed out of it. The
patient was arousal at the scene, and intubation was deferred because the patient was maintaining his
own airway. On arrival at the emergency department the patient was mildly trachcardic, with tem 38’c
and pupils 2mm. you are now called to assess the patient when a nurse notices a change in his cardiac
rhythm that has not responded to his medications.
