Instructions for Recipient


Box 1. Shows the total unemployment compensation paid to you in the calendar year shown. Some payments may
have been for weeks occurring in a prior year. NO adjustments have been made for overpayments repaid by you. This
information is being furnished to the Internal Revenue Service and the Oregon Department of Revenue. Combine the
box 1 taxable amounts from all Forms 1099-G, and report it as income on the unemployment compensation line of your
tax return.
Boxes 2 and 3. Not used.
Box 4. Shows the total federal taxes withheld for the calendar year. If you had no federal withholding you may request
that federal taxes be deducted by contacting an Unemployment Insurance Center. This information is being furnished
to the Internal Revenue Service and the Oregon Department of Revenue. Include this amount on your income tax
return as tax withheld.
Box 5. Shows alternative trade adjustment assistance (ATAA) and/or reemployment trade adjustment assistance
(RTAA) payments you received. Include on Form 1040 on the "Other income" line. See the Form 1040 instructions.
Boxes 6,7,8 and 9. Not Used.
