Training Needs Assessment Form Survey Questionnaire for Teaching Personnel

Directions: Rate each item as listed in the Questionnaire on the scale of I to 5. The first column
of ratings is about how urgent or important you think in this particular area. The
second column asks about the degree at which you feel or observe how have you
already acquired the necessary knowledge or skills in relation to the given area.

By doing such rating, the Training Needs Assessment method would be able to
determine the Measure of Training Needs by multiplying the needs urgency level.
The accomplished TNA Form will be analyzed by the Program Holders.

There are five (5) areas of training being assessed vis-à-vis with the PPST-RPMS, namely:
