    hereby appoint, name and constitute ______________________, of legal age, single and a resident
    of _________________________, Philippines, to be my true and lawful attorney-in-fact, for me and
    in my name, place and stead, to do and execute all or any of the following acts, deeds and
    things, to wit:

    1. To represent me in all transactions for the application of temporary


    electrical connection at Cagayan Electric and Power Co. (CEPALCO) for
