1. Valid passport which expires no less than 30 days after the expiry of the
intended date of departure from the Republic
2. A duly completed application form signed by the applicant. To be completed in
black ink only.
3. Two recent passport size photographs.
4. A yellow fever vaccination certificate, where applicable.
5. A medical report
6. A radiological report
7. Full birth certificate (certified copy).
8. Marriage certificate or in the case of a foreign spousal relationship, proof of
official recognition thereof issued by the authorities of the foreign country of the
applicant (where applicable)
9. The affidavit where a spousal relationship of a South African citizen or resident
is applicable, as well as documentation proving cohabitation and the extent to
which the related financial responsibilities are shared by the parties and setting
out the particulars of children in the spousal relationship.
10. Divorce decree, where applicable.
11. Court order granting full or specific parental responsibilities and rights, where
applicable.
12. Death certificate, in respect of late spouse, where applicable.
13. Written consent from both parents and full parental responsibilities and rights
parent, where applicable.
14. Proof of adoption, where applicable.
15. Legal separation order, where applicable.
16. Police clearance certificates in respect of applicants 18 years and older, in
respect of all countries where person resided for one year or longer since having
attained the age of 18.
17. A written undertaking by the employer accepting responsibility for the costs
related to the deportation of the applicant and his or her dependent family
members, should it become necessary. Letter to have company stamp.
18. A written undertaking by the employer to ensure that the passport of his or her
employee is valid at all times for the duration of his or her employment. Letter
to have company stamp.
19. A confirmation, in writing, from the professional body, council or board
recognized by SAQA in terms of section 13 (1) (i) of the National Qualification
Framework Act, or any relevant government Department confirming the skills
or qualifications of the applicant and appropriate post qualifications experience.
20. If required by law, proof of application for a certificate of registration with the
professional body, council or board recognized by SAQA in terms of section 13
(1) (i) of the National Qualifications Framework Act.
