LETTER OF AUTHORIZATION

Dear Cebu Pacific Air,

[I/We], the undersigned, hereby authorize Mr./Ms. ____________________________________


(“Authorized Representative”) to process and claim the refund of booking reference number
________________ amounting to ____________________ on [our/my] behalf (the “Booking”). The
amount of the booking will be deposited to Account Number/s: _________________ under the Account
Name of ___________________________, as reflected as well in the Bank Details Form submitted
together with this Letter.

[I/We] hereby confirm that Cebu Pacific Air shall be dealing directly with the Authorized Representative
for the purposes stated above and [I/we] give consent on any or all acts and documents executed by the
Authorized Representative in connection with this authorization. [I/We] also confirm that [I/we] shall have
no further action or claim against Cebu Pacific Air once the refund amount has been deposited in the
account detailed above, and that at the time of such deposit, this Letter shall serve as full waiver of any
and all claims [I/we] may have in connection with the Booking stated in the preceding paragraph.

signature for this Letter.]1
