    AFFIDAVIT OF CONSENT OF GUARDIANSHIP

    I, JOSE VILLACIN YAP, of legal age, married, Filipino and a


    resident of Bajada Davao City Philippines after being duly sworn to, in
    accordance with law, do hereby depose and say:

    1.      That I am the brother of Manuel Ladislao Villacin Yap who was


    previously under the care and custody of our other brother Raymundo
    Villacin Jr.;

    2. That after the death of our brother Raymundo Villacin Jr., Manuel Ladislao
    Villacin Yap has no more custodian responsible to take care of all his needs
    for his well being;

    3. That I am already eighty two (82) years old, in an advance age and with
    current physical disabilities, hence, it is very difficult for me to assume the
    responsibilities of taking care of my other older brother Manuel Ladislao
    who had a mental health issue;

    4. Therefore, I decided that it is our other brother Jesus Peter Villacin Yap who
    will take physical custody of him and to act as his guardian;

    5. That I execute this Affidavit inorder to manifest my consent that it will be


    our brother Jesus Peter Villacin Yap who will take physical custody of our
    other brother Manuel Ladislao Villacin Yap and to act as his guardian and
    for whatever legal purposes that this Affidavit may serve.
