PREFACE

Director (Refinery) of erstwhile Cochin Refineries Limited (CRL) appointed a committee in 1994 for
devising a procedure of standard operations. The committee formulated a system named Refinery
Operating System (ROS). A set of ROS was formulated and released by the committee. Over years,
the ROS was evolved to a well-documented Refinery Operating System (ROS), which outlines the
Management System Procedure for various business processes in the refinery. There were 90
approved ROS and 12 ROS under formulation before the current edition. Prior to 2015 edition,
Project (Technical) department, which was earlier known as Corporate Planning, before
organizational re-structuring in 2006, was the custodian of the ROS.

In 2014, General Manager (Operations) constituted a task force for a thorough revision of ROS in
terms of the purpose, relevance, scope and work flow in the current business context vide office
note KDD/03/09 dated 28-02-2014.

The previous version of this ROS was ROS-94 Rev0. The task force revised the previous revision with
new structure in line with ROS-001 and renumbered as ROS-020 as per the new ROS index.

The procedures / practices described in the ROS are not intended to supplant other practices /
procedure that have proven satisfactory, nor is the ROS intended to discourage innovation and
originality in the best practices in the refinery. Users of the ROS are reminded that no ROS is a
substitute for the judgment of a responsible, qualified person.

1 INTRODUCTION

Working at height is unavoidable for operations, maintenance and construction in refinery


and hence, temporary platforms and scaffolding are provided for the safe working of
personnel at height. Scaffold is a temporary safe work platform, either supported from
below or suspended from above, on which workers can perform tasks at heights above the
ground when work cannot be done at ground level or on a finished floor. Scaffolds, once
properly erected, are a control measure to prevent risk of persons and objects falling while
working at height.

Working at height remains one of the biggest causes of fatalities and major injuries in the
workplace. Common cases include falls from ladders and through fragile surfaces. If there are
no precautions in place, a person may fall a distance liable to cause personal injury and
hence control measures are inevitable.

1.1. PURPOSE

The purpose of this ROS is to establish the minimum safety guidelines for scaffolds used in
BPCL Kochi Refinery.

1.2. SCOPE

The scope of this ROS is to establish procedure for safe erection, use, and dismantling of
scaffolds for operations, maintenance, and construction activities by BPCL or by contractors
at facilities of KR and project sites.

1.3. COVERAGE

ROS is applicable to the following facilities under KR SBU

1. Kochi Refinery

2. Jetty Facilities like NTB, STB, & COT

3. Pipeline corridor

4. STF and SPM

5. SWSS Aluva

1.1 LIMITATIONS

This ROS shall not be used in conflict with legal and other requirements applicable to KR for
erection of scaffold and working at height. However, if the requirements of this ROS are

more stringent than the requirements of the above, then the requirements of this ROS shall
govern.

1.2 EXCLUSIONS
1. Other SBUs of the Corporation.

1.3 DEVIATIONS

The procedures / practices described in the ROS are intended for standardization of the
processes involved and hence the same shall be strictly adhered to. However, deviation, if
any, arises due to unavoidable circumstance(s), shall be approved by Head of Kochi Refinery.
The deviation note in the prescribed format specified in ROS-001 Appendix ‘ROS-001/A9.4:
Format for Approval of Deviation in ROS’ shall be initiated by the HOD of the department
proposing the deviation through the Functional Head.

2 DEFINITIONS & ABBREVIATIONS

This section lists all definitions and abbreviations used for the purpose of this ROS.

2.1 DEFINTIONS
Base Plate : A plate for distributing the load from an upright.

Bay : The space between the centre lines of two adjacent standards
along the face of the scaffold.

Brace : A tube incorporated diagonally in scaffolding for stability.

Corporation : Bharat Petroleum Corporation Limited.

Coupler : A fitting by which a grip is applied to the external surfaces of two


tubes that thereby holds them together.

Functional Head : Head of Functional Area such as General Manager, Deputy


General Manager (In charge).

Functional Area : Operations, Technical, Finance, Human Resources, Projects etc.

Guardrail : A tube connected at the edge of platform and other places to


prevent persons falling from platform or place. It is also known as
a Handrail.

Lanyard : The line used to connect the safety harness to anchorage point.

Ledger : A tube spanning horizontally and tying a scaffold longitudinally


that may act as a support for putlogs or transoms.

Lift : The height from the ground or floor to the lowest ledger or the
vertical distance between any two adjacent ledgers.

May : Optional

Putlog : A tube or other member spanning from a ledger to the wall of a


building that may have a specially formed end (may be
detachable) for the purpose of fixing into the brickwork.

Raker : An inclined tube having a bearing on the ground or on an adjacent


structure or if carrying portion of the projecting scaffolding being
connected to a horizontal member of the scaffold at a lower level.

Refinery : Kochi Refinery premises and facilities under its administrative


control.

Safety harness : Personal Protective Equipment consists of waist belt together


with shoulder straps and leg straps.

Shall : Mandatory

Should : Recommended

Standard : A tube used as a column or vertical in the construction of a


scaffold, and transmitting a load to the ground via a base plate. It
is also known as an Upright or Column.

Transom : A tube spanning across ledgers to tie a scaffold transversely that


may also support a working platform.

3 REQUIREMENTS

This section of ROS covers hazard identification and risk assessment, types, requirements for
materials used, safety requirements in erection and dismantling of scaffolds.

3.1 HAZARD IDENTIFICATION AND RISK ASSESSMENT

Since working at height involves various hazards and care shall be taken to identify the
hazards and assess the risk to minimise untoward incidents.

3.1.1 IDENTIFYING THE HAZARD

Some examples of the hazards associated with work involving the erection, use,
maintenance, alteration and dismantling of scaffolds include:

a. Scaffolding collapse (before, during and after placement of the scaffold)

b. Overhead electric lines

c. Mobile plant and other workplace traffic

d. Fall from heights

e. Falling objects

3.1.2 ASSESSING THE RISK

Risk assessment shall be carried out to select the most suitable type of scaffold for the work
and minimise number of persons working at heights for erecting, use and dismantling
scaffolds. The following aspects are to be considered while carrying out the risk assessment.

a. type of scaffold to be used

b. height of the scaffold to be erected

c. scheduling of the scaffolding work

d. layout of the workplace, including proximity to public areas

e. surface on which the scaffold will be erected (ground conditions, the structural integrity
of the surface to support the scaffold and its load)

f. number of people intend to work on the scaffold

g. machinery that will be used on the scaffold, plant and equipment near the scaffold

h. skill and competencies required to erect, use, maintain, alter and dismantle the scaffold

i. local weather conditions, particularly wind forces

j. type of work

3.2 TYPES OF SCAFFOLD

Various types of scaffolds are used based on the job requirements and some of the
commonly used ones are described below.

3.2.1 BIRD CAGE SCAFFOLD

A birdcage scaffold is an independent scaffold that consists of more than two rows of
standards in both directions and is connected by ledgers and transoms. It is mainly used for
working on a single level, such as ceilings.

3.2.2 TOWER SCAFFOLD

A tower scaffold is an independent scaffold consisting of four vertical members connected


longitudinally and transversely.

3.2.3 MOBILE SCAFFOLD

A mobile scaffold is a tower scaffold that is mounted on castors as shown in Figure- 1.

3.2.4 TUBE AND COUPLER SCAFFOLD

Tube and coupler scaffolds are so-named because they are built from tubing connected by
coupling devices. They are frequently used where heavy loads need to be carried, or on
structures with unusual design, shape and locations because of their strength. The versatility

of tube and coupler scaffolds enables them to be assembled in multiple directions in a


variety of settings.

3.2.5 PRE-FABRICATED FRAME SCAFFOLD

Frame scaffold is a scaffold assembled from prefabricated frames, braces and accessories.
Frame scaffolds such as ‘A’ and ‘H’ frame trestle scaffolds are commonly used by bricklayers,
plasterers and painters, and for general fit-out and finishing work.

3.2.6 HUNG SCAFFOLD

A hung scaffold is an independent scaffold that hangs from another structure, but is not
capable of being raised or lowered when in use.

3.2.7 SINGLE POLE SCAFFOLD

A single pole scaffold consists of a single row of standards connected by ledgers. Putlogs are
fixed to the ledgers and built into the wall of the building or structure. A single pole scaffold
is dependent upon the structure against which it is placed for support. It is important that no
components of this type of scaffold are removed until the scaffold is being dismantled.

3.2.8 SUSPENDED (SWING-STAGE) SCAFFOLD

A suspended scaffold incorporates a suspended platform that is capable of being raised or


lowered when in use. An example of a suspended scaffold is a swing-stage scaffold.

3.3 GENERAL REQUIREMENTS

1. Scaffold shall be erected in accordance with the guidelines specified in this ROS for
persons engaged in work at height that cannot be done safely from the ground so as to
prevent fall of person and material from height.

2. Scaffolds shall be provided with safe means of access, such as stairs or ladders. Ladders
shall be secured against inadvertent movement.

3. Every scaffold shall be properly designed, constructed, erected and maintained so as to


prevent collapse or accidental displacement.

4. Every scaffold and parts thereof shall be

a. so designed as to prevent hazards for workers during erection and dismantling.

b. so designed that guardrails and other protective devices, platforms, putlogs, rakers,
transoms, ladders, stairs or ramps can be easily put together.

c. of suitable and sound material and of adequate size and strength for the purpose
for which it is to be used.

d. maintained in a proper condition.

5. Scaffolds and their components shall be capable of supporting without failure at least
four times maximum intended load.

3.4 MATERIAL OF CONTRUCTION

1. Scaffolds tubes are to be made from heavy class welded or seamless steel tubes of
40mm nominal bore conforming to IS: 1161-1998.

2. Steel tubes for individual component type scaffolding shall be of 6ft., 8ft., 10ft., 12ft.,
14ft., 16ft. and 20ft. in length and may be either galvanized or plain. Plain steel tube
shall be protected against corrosion by applying two coats of zinc chromate primers or
zinc phosphate primer followed by two coats of chlorinated rubber based finish paint.
The plain ends of tubes shall be chamfered off preferably to 3mm.

3. Straight clamps should be used to connect two tubes at 90 degrees. Cross clamps or
swivel camps should be used to connect two tubes at an angle.

4. Bolts and Nuts used shall be of mild steel / black bolt confirming to IS: 1362.

5. Scaffold steel boards shall be lightweight, sturdy and rugged construction preferably
300mm wide, 2000 to 2500mm long maximum and free from any defects likely to affect
its strength. Steel board gratings shall be designed such a way that no tools shall fall
through it.

6. Wooden boards shall not be used to substitute of steel boards.

7. When steel boards are used, use wire rope for tying the boards or alternate safe
method may be used.

8. Rope made of plastic, coir etc. shall not be used for erection of scaffolds and for tying
scaffold boards.

9. Materials used in the construction of scaffolds should be stored in good condition and in
a manner that shall not affect the integrity of the scaffold materials.

10. All tubes, couplers and fittings used for scaffolds shall be free from damage and
distortion and shall be maintained in rust free condition.

11. Couplers shall be made of drop forged steel or equivalent material.

3.5 ERECTION OF SCAFFOLD

1. All unwanted materials shall be removed from the area before erection of scaffold.

2. Ensure the ground is firm where the scaffold is intended to be erected. When scaffold is
to be erected in the vicinity of excavation or on low earth, ensure that standards are not
likely to slide or cave in.

3. The bay length is dependent upon the height and load on the scaffold but shall never
exceed 2500mm. All base lift shall be supported on metallic base plates of adequate size
and must be erected straight.

4. Except on the side adjacent to the site of work all standards shall extend at least
1050mm above the top most working platform to enable handrail / guard rail to be
fitted. Guardrail, mid rail and toe rail may be fitted 1050mm, 525mm and 75mm
respectively above the platform level across the scaffold structure.

5. The vertical distances between ledgers shall not be more than 2100mm.

6. The horizontal distance between transoms and putlogs carrying 32mm thick boards
shall not exceed 1500mm.

7. The gap between front edge of platform and work shall not be more than 350mm.

8. The area within toe guard and handrail shall be fully covered using scaffold boards.
There shall not be any gap between the scaffold boards.

9. The end of the scaffold board shall not extend more than 300mm or less than 150mm
beyond the transoms.

10. The ledgers shall not extend more than 250mm beyond the standards at the ends. The
ledgers up to a height of two metres shall be provided with rubber end covers to
prevent injury to people moving in the area.

11. Safety harness shall be used while working at height above 2m and it shall be suitably
anchored to limit the fall distance to minimum and also to avoid falling on to the
ground.

12. Every scaffold shall be effectively braced to make it rigid and tied to make it stable.
Scaffolds may be effectively tied to adjacent structure to ensure stability.

13. A safe and convenient means of access shall be provided to the platform level of a
scaffold. Portable ladders may be used for this purpose up to a height of 4 m. When

used, it shall be placed at an angle of approximately 75 degrees from the horizontal. For
platforms above 4 m height, fixed ladders shall be provided and tied or hooked with
scaffold to take adequate load of people.

14. Wherever provision of a ladder is not feasible, provide additional rungs at a spacing of
450mm at any convenient side of the scaffold.

15. Ensure that no un-insulated overhead electric lines exist within 3m of the working
platform.

16. Scaffolds, which are not designed to be independent, should be rigidly connected to a
structure or a building at suitable vertical and horizontal distances.

17. A scaffold shall never extend above the highest anchorage to an extent, which might
endanger its stability and strength.

18. Sufficient putlogs and transoms shall remain in position and securely fastened to the
ledgers, uprights or standards as the case may be, to ensure the stability of the scaffold
until it is finally dismantled.

19. All scaffolds and appliances used as supports for working platforms shall be of sound
construction, have a firm footing and be adequately braced to maintain their stability.

20. When necessary to prevent danger of falling objects, overhead screens / safety net of
adequate strength and dimensions shall be provided for working platforms and
stairways of scaffolds.

21. Scaffolding materials shall not be thrown from scaffolds or from heights. Other
materials may be thrown (if material is not affected by the action) from scaffolds or
heights only when the landing areas has been designated, protected, appropriate notice
displayed and is under the supervision of a person on the landing level.

22. Platforms on scaffolds shall be of adequate dimension, especially in width, for the tasks
to be performed from the scaffold.

23. Resting platforms shall be provided at every six meter heights for high rise scaffolds.

24. Utmost care is to be taken against fall while erecting a scaffold. Use of full body safety
harness with double lanyards is recommended to ensure that at least one is always
anchored at all times even when moving up or down.

25. Only trained persons shall be allowed to erect scaffolds with valid work permit.

26. During erection of scaffold a red coloured “DO NOT USE SCAFFOLD” tag as given in
Appendix ROS-020/A9.2 shall be displayed on the scaffold for preventing usage of
incomplete scaffold.

3.6 PRE-FABRICATED SCAFFOLD

1. Prefabricated frame scaffold should be erected and dismantled by a competent person


in a progressive manner to ensure both the installers’ safety and the stability of the
overall structure.

2. Braces shall be attached to the frames in accordance with the manufacturer’s or


supplier’s instructions. As the height of frames increase, there is a greater need to
provide lateral stability to the frames.

3. Prefabricated frame scaffold shall be stable and erected on a suitable foundation to


ensure that it can adequately carry and distribute the loads such as materials and
workers evenly to each frame.

3.7 SUSPENDED SCAFFOLD

In addition to the requirements for scaffold in general as regards to soundness, stability and
protection against the risk of falls, suspended scaffolds shall meet the following specific
requirements:

1. Two point suspension scaffold platforms shall be not less than 500mm and no more
than 914mm wide overall. The platform shall be securely fastened to the hangers by U-
bolts or by equivalent means.

2. The hangers of two point suspension scaffolds shall be made of wrought iron, mild steel,
or other equivalent material having a cross-sectional area capable of sustaining four
times the maximum intended load.

3. When hoisting machines are used on two-point suspension scaffolds, such machines
shall be design tested and approved by a competent person.

4. Wire or fibre rope used for scaffold suspension shall be capable of supporting at least six
times the intended load.

5. The safety of workers shall be safeguarded by an extra rope having a point up


attachment independent of the anchorage arrangements of the scaffold

6. The anchorage and other elements of support of the scaffold shall be designed and built
in such a way as to ensure sufficient strength.

3.8 INTERNAL SCAFFOLD

1. While erecting scaffold inside heaters, vessels, columns, the Engineer in Charge of the
job may grant relaxation in width and height of scaffold to be erected due to space
constraint. However fitness of scaffolds shall be ensured and certified. Safety harness
shall be used at heights above 2 m and same shall be suitably anchored.

2. Provide platform all around at one meter below the tube clamp level while erecting
scaffold inside heaters.

3.9 INSPECTION AND MAINTENANCE OF SCAFFOLD

1. After erection, the Engineer in Charge of the job shall inspect and ensure construction of
the scaffold as per the plan.

2. The Engineer in Charge of the job shall inspect the scaffolding as per the checklist given
in Appendix ROS-020/A9.1. He /she shall certify the scaffolding fit for use and provide
the duly filled green ‘READY FOR USE SCAFFOLD’ tag with his / her signature given in
Appendix ROS-020/A9.2. The Red coloured ‘DO NOT USE SCAFFOLD’ tag shall be
removed from the scaffold, which was put during erection of scaffold.

3. No person shall be allowed to work on scaffolds during storms or high winds. After
heavy rains or storm the Engineer in Charge of the job shall inspect scaffolds; ensure
that the integrity is not lost and certify once again.

4. A scaffold once erected shall not be substantially altered or dismantled without the
concurrence of the executing Engineer in Charge.

5. The validity of an approved scaffold is up to 7 days. Thereafter the scaffold shall be


revalidated by the Engineer in Charge of the job using the checklist given in Appendix
ROS-020/A9.1 and renew the certification by signing on that renewal column in the tag.

6. No scaffold should be partly dismantled and left with ‘READY FOR USE SCAFFOLD’ tag.
In case the scaffold is to be used for other works (e.g. using the partially dismantled
scaffold with lower height), it shall be used only after recertification by the Engineer in
Charge.

3.10 SAFETY DURING USAGE OF SCAFFOLD


1. Ensure that scaffolds are used appropriately and only for the purpose for which they are
designed or erected.

2. In transferring heavy loads on or to a scaffold a sudden shock to the scaffold shall be


avoided.

3. When necessary to prevent danger, loads being hoisted on or to scaffolds should be


controlled by a guide line so that they don’t strike against the scaffold.

4. The load on the scaffold should be evenly distributed, as far as practicable, and in any
case should be so distributed as to avoid disturbance of the stability of the scaffold.

5. During the use of a scaffold care should constantly be taken that it is not overloaded or
otherwise misused.

6. Scaffold should not be used for the storage of material except that required for
immediate use.

4 ROLES AND RESPONSIBILITIES

This section outlines only the major roles and responsibilities of various stakeholders related
to the ROS-020. The roles and responsibilities are detailed in the requirement given in
Section of 3.0 of this document. The stakeholders shall comply with these requirements.

4.1 ENGINEER IN CHARGE


1. Ensure that the contractor erecting the scaffold is putting a red tag on the scaffold
giving warning “DO NOT USE SCAFFOLD” during erection.

2. Ensure that the scaffold erected is strictly as per the guidelines of ROS-020.

3. Inspect the scaffold after erection as per the checklist given in Appendix ROS-020/A9.1
and certify the scaffold ready for use by replacing the red tag “DO NOT USE SCAFFOLD”
with the green tag “ READY FOR USE SCAFFOLD” with signature duly endorsed.

4. Inspect the scaffold after heavy rain or storm and ensure that the integrity of the
scaffold is not lost.

5. Revalidate the scaffold in every 7 days using checklist and by signing on the renewal
column of the green “READY FOR USE SCAFFOLD” tag.

5 APPROVAL

This ROS undergoes a review and approval process in line with the process specified in
Section 5.0 of ROS-001.

5.1 REVIEW OF ROS


1. HODs of F&S, Maintenance and E&C and Functional Heads of Operations and Projects
are the reviewing authorities of this ROS.

2. The reviewing authorities shall ensure that requirements of ROS are relevant with
respect to context of the business and are in line with current environment.

3. The reviewing authorities shall sign on the Document Release Note of this ROS.

5.2 APPROVAL OF ROS

Refinery Council (RC) is the approving authority for the approval of this ROS. Head of KR is
the signatory on behalf of the RC and shall sign on the Document Release Note of this ROS.

6 MONITORING AND CONTROL


1. Fire & Safety department shall conduct periodic visits to the site and verify compliance
with this ROS. Any deviation shall be reported in the operations meeting by HOD of fire
and safety department for necessary corrective action.

2. The compliance of this ROS shall also be verified during the periodic safety walk of
senior executives and non-compliance shall be reflected in the report generated.

3. The compliance shall also be ensured during the internal safety audit.
