    AFFIDAVIT OF CORRECTION OF ENTRY

    I, JEANETTE R. CUICO, of legal age, married, Filipino citizen and a resident


    of Mampang, Zamboanga City, under oath, do hereby depose and state-

    1. That I am the surviving daughter of the late FELICITAS ENEBRAD RAMOS


    who died on August 16, 2020, in Zamboanga City;

    2. That I was the one who processed her Certificate of Birth before the
    Philippine Statistics Authority;

    3. That on the process of registering her death, I erroneously declared that


    her date of birth is on December 16, 1952 instead of December 16, 1951 as
    reflected in her Certificate of Live Birth;

    4. That I am executing this affidavit to attest the veracity of the foregoing facts and
    for all intents and purposes this may serve.

    IN WITNESS WHEREOF, I have hereunto set my hand this 1 st day of September,


    2020, in the City of Zamboanga, Philippines.
