1.0 INTRODUCTION

The purpose of this Method Statement is to define the procedure for Site Surveying and Setting out of
Works to ensure that the implementation at the site is complying with the condition & requirements as
stipulated in the specifications.
1.2 Scope
The scope of this Method Statement covers all procedures for Setting Out of works as specified in each
designated location of the approved drawing.
1.3 Required Permits
Permits will be issued as per project requirements and QCS2014.

2.0 RESPONSIBILITIES
2.1 Project Manager
 Management body of the methodology to contact and maintain with the QA/QC Department, HSE
requirements and company recommendations;
2.2 Construction Manager
 He shall be responsible in overall project execution. He will also monitor his subordinates and ensure
that all activities will be completed with respect to safety, quality, time, and cost.
2.3 Project Engineer
 He shall assign areas of responsibilities to Site Engineers.
 He shall be monitoring and controlling project execution with respect to safety, quality, time, and
cost.
 He shall be coordinating between different function with the project affecting the activity.
 He shall be expediting areas of concern to achieve project schedule.
2.4 Site Engineer
 He shall assign areas of responsibilities to Site Supervisors
 He shall be monitoring and controlling project execution at site with respect to safety, quality, time,
and cost.
 He shall be responsible for proper handling, storage, and installation of any approved material in line
with this method statement, specifications and drawings; and in reporting any possible deficiencies
encountered.

 He shall be responsible in liaising with QA/QC Engineers / QC Inspectors regarding work progress
and hold point inspection.
2.5 QAQC Engineer
 He shall advise Petroserv staff and management on ways to fulfil the Quality requirements
mentioned in this document.

 He shall prepare and develop a process of monitoring the procedure mentioned in this document to
insure it is strictly followed and will continue to evaluate its effectiveness and revised as necessary
for quality improvement.
 He shall conduct an awareness training to personnel directly involves this method to impart a proper
understanding of its requirements
 He shall conduct an internal audit to his staff and shall also 2nd Party Audit to the supplier to ensure
the Quality Control mentioned in the prequalification document is being followed.
 He shall support the QAQC Engineer on quality issues in the site regarding the implementation of
work.
2.6 QC Inspector
 He Shall carry out the material inspection to ensure that they are as per the approved material
submittal and they meet the desired quality standard upon receipt
 He shall ensure that the Inspection and Test Plan (ITP) is briefed to and are being followed by the
construction team.
 He shall prepare required quality documentation and its approval.
 He shall initiate and maintain check sheets / records for works.
 He shall liaise with Site Engineer regarding work progress and for hold point inspection.
 He shall expedite areas of concerned to best achieve project quality requirements.
 He shall do real time monitoring of on-going work activities.
 He shall prepare and update the status reports of work progress inspection.
2.7 Health and Safety Manager
 He shall advise Petroserv staff and management on ways to fulfil the Health and Safety requirements
mentioned in the Health and Safety Plan.
 He shall support the Site Safety Representatives on health and safety related issues.

 He shall carry out Health & Safety Hazard Impact / Risk Assessment for the site commencement of
work.
 He shall carry out inspections and raises Health & Safety NCR’s for Non-conformities with related
activities.
 He shall ensure work site is in safe conditions.
 He shall ensure calibration & approval of all Health and safety monitoring Equipment and ensure
needed certifications for lifting equipment, machineries, Slings etc.
 He shall ensure First aid and Health and Safety Emergency Procedures at site to know the personnel
working for this activity.

2.8 Safety Officer


 He shall be reporting to the Project/Safety Manager and will be fully responsible for Health and
Safety requirements of the project. He will be conducting all required tool box talks, safety reports
and make sure that all the safety procedures are meeting the requirements.
 He shall ensure that required Personal Protective Equipment (PPE) is worn at all times.
 He shall ensure all workers / technicians are briefed about hazards associated with this activity.
 He shall be monitoring workers and worksites for any safety related issues.
2.9 Surveyor
 He shall be responsible in assuring continuous follow up on levels and alignment of the work.
 He shall be responsible to ensure all survey tools and equipments shall be calibrated by an approved
third party calibration laboratory.
 He shall be responsible in preparing survey report and to get the approval of Consultant’s Surveyor
upon completion of joint inspection.
2.10 Site Supervisor
 He shall assign areas of responsibilities to Foreman.
 He shall be responsible in executing daily activity plan at site as per approved method statement and
shop drawings.
 He shall be responsible provide current status of work to concerned site engineers.

2.11 Foreman
 He shall be responsible in executing a specific work at site ensuring all activities are as per approved
method statement and shop drawings.
2.11 Workers
 Responsible in wearing suitable P.P.E, following safety procedures and to do the work as described
in the method statement.
2.12 Machine / Equipment Operator
 Responsible in operating the machines and equipments as per instruction of Site Engineer or Site
Supervisor complying necessary permits prior to commencement of the activity.
 Perform periodical safety, maintenance or servicing checks to ensure proper functioning of the
equipment in a safe manner avoiding accidents and damage of property.
2.13 Independent Testing Laboratory
 Responsible to carry out the necessary testing of the materials used and report to the QC Team.

The combination and the number of equipment will be determined in the mobilization plan depending on
site condition such as works area, work type and work quantity in each work stage.

4.0 HEALTH AND SAFETY


Petroserv shall fully adhere about project HSE plan, and that stated all requirements fully implemented in the
site during execution of work.

4.1 Workforce protection & provision of access:


 Petroserv will provide sustainability awareness thru induction and training of all personnel prior to
discharging them of their duties and responsibilities in the site.
 Worker Clothing-Workers exposed to traffic should be attired in bright, highly visible clothing
similar to that of flaggers.
 Barriers-Barriers should be placed along the work space depending on such factors as lateral
clearance of workers from adjacent traffic, speed of traffic, duration of operations, time of day, and
volume of traffic.

 Speed Reduction-In highly vulnerable situations, consideration should be given to reducing the speed
of traffic through regulatory speed zoning, use of police, lane reduction, or flaggers. Site speed limit
shall not be exceeding 30 km/ph.
 Lighting-For night-time work, lighting the work area and approaches may allow the driver better
comprehension of the requirements being imposed. Care should be taken to ensure that the lighting
does not cause blinding.
 Public Information-Improved driver performance may be realized through a well-prepared and
complete public relations effort that covers the nature of the work, the time and duration of its
execution, and its anticipated effects upon traffic and possible alternate routes and modes of travel.
Such programs have been found to
result in a significant drop in traffic; that reduces the possible number of conflicts and may allow a
temporary lane closing for additional buffer area.
 Road Closure-If alternate routes are available to handle detoured traffic, the road may be closed
temporarily during times of greatest worker hazard-which, in addition

to offering maximum worker safety, may facilitate quicker project completion and thus further reduce
worker vulnerability.
 Access segregating the work zone and vehicular access shall be provided.
4.2 Movement of vehicle on site:
 Establishing a site speed limit;
 Banks man shall be deployed for vehicle movement in site working areas.
 Safe parking of staff, contractor, tenants, facility user, and visitor vehicles on or adjacent to site;
 Loading and unloading of vehicles to be controlled by flagmen.
 Maintaining access for emergency service vehicles at all times to all areas of the site.
 Unauthorized access of vehicles are to be controlled by site security personnel.
 Deliveries are supposed to be done in Site store yard rather than in worksite, However in certain cases
security personnel will control the deliveries on site.
4.3 Traffic Management:
Strictly implement at site approved traffic management plan and approved TDP
4.4 Emergency Response
 Safe Assembly Point (Muster Point) is located in near entrance of each site location.
 In case of an emergency, the work force will assemble in the safe assembly point in consideration to
the wind direction (safe assembly point is provided with sign boards)

 Time keeper / security shall carry the attendance register of specific location for head counting of
attendees.
 Safety officer shall verify the attendance register to ensure that all the personnel’s have evacuated
from the hazard area.
 Attempt to put out the fire / rescue operation shall be made only when it is safe to do so. Fire shall be
put out under supervision of trained personnel.
 All flammable material in the vicinity of fire shall be removed.
 Safety engineer/safety officer shall call fire brigade, ambulance, police, etc..., and contact project
manager/HSE manager for further action.
 First aid shall be administered to the injured personnel if any.
 Standby vehicle and driver shall be available in case of emergency.

 Project manager shall ascertain the risks and ensure control measures are implemented prior to
resuming work.

4.7 CONFINED SPACE:


 A confined space is any enclosure having a limited means of entry and exit and not designed for
continuous human occupancy. It includes storage tanks, underground pipelines, man holes, process
tanks and equipment’s.
 Confined space entry is defined as the act of physically entering a confined space. Entry covers not
only complete body entry, but also when only the head is inserted for a quick visual inspection.

 Entry into the confined can expose personnel to one or more of the following hazards:
a) Hazardous atmosphere such as toxic or flammable vapors.
b) Oxygen-deficient atmosphere.
c) Material such as sludge.
d) An internal configuration such that the entrant could be trapped or asphyxiated.

e) Other air contaminants including hydrocarbon vapors or gases, vapors resulting from materials
used in the confined space etc.
f) Physical hazards e.g. slippery surfaces that may hinder rescue operations, falling tools and
objects, poor structural integrity etc.
g) Under no circumstances shall an employee be allowed to enter a confined space without a
properly completed and approved Permit-to-work. The Work Permit shall have a sign-in/Sign-
out sheet for entrants and the hazards shall be discussed with entrant prior to that person entering
the space. Pre entry safety meeting shall be held.
h) All employees who will enter confined space shall be trained in entry procedures. Personnel
responsible for supervising, planning, entering, or participating in confined space entry and
rescue shall be adequately trained in their functional duties prior to any confined space entry.
i) PPE, gas mask, eye protection and helmets shall be worn by the personnel while working in
confined space. If required use Breathing Apparatus.
j) HSE representative trained in use of gas level monitor shall enter the confined space with
protective clothing, PPE, Breathing Apparatus on for testing gas level inside the confined space
with an approved and calibrated device.
k) GAS test can also be conducted and continuously monitored if requires.
l) Ensure periodic atmospheric monitoring is done according to permit requirements.
m) HSE Representatives and foremen’s shall be certified and trained in Confined Space Entry. HSE
Representative shall also ensure that all entrants have been trained on confined space entry
procedures.
n) Provide Toolbox talk to the workers involved in the confined space.
o) One or more trained persons shall be assigned as a standby man for confined space entry. There
shall be an effective means of communication established and maintained between confined
space entrants and the standby man. Communication shall be visual, voice or a signal line.
p) The standby man shall not leave his duty post for any reason while the entrant is in the confined
space unless the standby man’s replacement is already in position.

q) Standby man shall be stationed outside of the confined workspace and he shall maintain a sign-
in/sign-out log with a count of all persons in the confined space, and ensure all entrants sign in
and out. Standby man shall call immediately to the rescue team in case of confined space
emergencies
r) Trained Rescue Team shall be assigned. Rescue team shall respond immediately to rescue calls
from the Standby man or any other person recognizing a need for rescue from the confined
space. It has to be ensured that the rescue team members shall have current certification in first
aid as well as confined space entry. They shall be well trained in usage of rescue equipment.
s) An emergency response plan shall be drawn up and well understood by all role players of the
rescue team.
t) Instruct the employees / standby man not to attempt any rescue operation if they see any person
collapsed in the confined space or no communication from the employees working in the
confined space unless they know the real situation (Risks and Hazards) and properly protected
against the hazard. Call immediately the assigned rescue team.
u) Instruct the employees / standby man that they should not take any step in emotions to save their
friends / colleagues during confined space emergencies. Ask them to call immediately to the
emergency rescue team.
v) Instruct the entrants to alert Standby man immediately when a prohibited condition exists and/or
when warning signs or symptoms of exposure exist.
w) Refresher training shall be conducted as needed to maintain employee competence in entry
procedures and precautions.
x) Instruct the employees / standby man to remember emergency contact numbers of the rescue
team.
y) Emergency equipment:
 Tripod with winch
 Safety harness
 Air blower
 Multi-gas detector
 First aid
 Breathing apparatus (will be kept on first aid station)

z) Accepted level of atmosphere:


a) Oxygen (O2) content between 19.5% and 23.5%.
b) Lower Explosive Limit (LEL) less than 10% of Lower Explosive Limit (LEL).

c) H2S – Below 10 ppm (above 10 ppm, entrants should wear Breathing apparatus, above
100 ppm, entry shall not be allowed).
d) Other toxic gases – Reading shall be below Threshold Limit Value (TLV) for entry
without air mask (E.g. CO – <25 ppm, Cl2 – <0.5 ppm, SO2 – <2 ppm, NH3 – <25 ppm.)
aa) For Hazardous atmosphere (Toxic Vapors, H2S, other flammable gases):
 If hazardous gas has been detected in the confined space, air purging shall be done.
Hazardous gases can be purged by using exhaust fans that shall be fitted to the shell in air
extraction mode and allowed to run continuously until the atmosphere in the confined
space is free of hazardous gases.
 Gas level shall be monitored continuously at fan outlet to the accepted level. If the air in
the confined space is still contaminated, the space shall be further degassed by purging
with air as necessary until an acceptable working atmosphere is achieved prior to a person
entering the space.
4.8 PERSONNAL PROTECTIVE EQUIPMENT (PPE):
Personnel protective equipment shall be provided by the company for all staff including visitors as per
stipulated in approved HSE plan and approved risk
assessment based on individual activity. The following PPE will be minimum mandatory at site:
 Reflective Safety Jacket
 Hard Hat
 Coveralls
 Safety Shoes/Boots
 Protective Eye Goggles
 Dust Masks
 Working Gloves

4.9 WELFARE FACILITIES:


a. Petroserv will provide adequate and appropriate welfare facilities for workers while they are at work.
This means must provide such facilities unless it is clearly unreasonable in terms of time, trouble,
cost and physical difficulty.
b. Adequate toilet and washing facilities for employees within the work locations.
c. Well-equipped rest shelter with proper lighting and ventilation.
d. Sufficient Drinking water and water cooler.
e. Every employee is subjected to take breaks in for breakfast and lunch.

f. Ensure that the facilities are kept clean and in good condition.
g. Petroserv shall provide first aid boxes furnished with the medicines, tools, and equipment. The first
aid box shall be available on site office and satellite offices.
4.10 SURFACE WATER AND GROUND WATER CONTROL:
 Petroserv shall maintain and operate dewatering continuously in 24 hours to exclude water from the
works while construction is in progress.
 Excavated areas shall be kept well drained and free from standing water.
 Disposal of ground water shall be as per approved method only.
 Subcontractor for dewatering will be submitted for approval as per QCS 2014 Section 8 Part 2
Clause 2.2.14.

5.0 SEQUENCE OF WORKS


5.1 Permits and Certification
Before commencement of excavation, all the necessary permits shall be acquired. In addition to the
required permits mentioned in Clause 1.3 of this document.
5.2 General
 Work is to be performed by experienced surveyor under the supervision of qualified supervisor who
is fully aware of the project requirements.
 An approved Third Party Surveying Company shall establish the level datum and benchmark from
Qatar National Datum.
 Survey records shall be handover to the contractor’s surveyor for checking of contract drawings.
 Temporary benchmark shall be available and shop drawings shall be approved before
commencement of any setting out activity
 All Setting Out Works must comply with QCS2014 Section 8 Part1 Clause 1.3.3

5.3 Site Information and Inspection


 Before commencing the setting out of the Works, surveyor shall ascertain the location of all existing
underground services within the site boundary. As-Built drawing will be asked to Engineer’s
Representative for the details of the location of the services.
 Any conflict between existing services and any part of the proposed Works shall be brought to the
attention of Engineer’s Representative.
 Before commencing the setting out of the Works, Petroserv and Engineer’s Representative shall
make an inspection of the Site.
5.4 Setting Out of Works

 Mark the center of proposed pipelines and manholes from the bench marks created by approved
survey company using spray paint or survey pegs and ensure the true and proper setting out of the
works in relation to reference data as per latest IFC drawings and shop drawings.
 To locate by co-ordinates to the Qatar National Grid, the center of all existing covers of manholes
and chambers constructed along the existing pipe lines.
 Setting out shall be reviewed by Engineer’s Representative before commencing the Works.

6.0 QUALITY CONTROL AND RECORDS


 All equipment shall be calibrated by an approved third party calibration laboratory.
 All workmanship shall be carried out by skilled people.
 All works shall be conducted as per IFC drawings, approved shop drawings, and
Method Statement.
 Inspections and Tests shall be done in compliance to the approved Inspection & Test
Plan (ITP).
RECORD
Contractor will supervise the Testing Agency and maintain records and reports in accordance with the
applicable codes and standards and shall keep a daily record of all the works carried out. As follow;
