    AFFIDAVIT OF SUPPORT

    I, PHIL ADNIL N. CALUMPANG, of legal age, married, Filipino, and residents of


    Pulangtubig, Dumaguete City, Philippines, after having been sworn to in accordance with the
    law, do hereby depose and say:

    1. That I am the husband of visa applicant MARIA RENEE SOL D.CALUMPANG;

    2. And the father of visa applicants, PHIL RENZO D. CALUMPANG and DEELAN D.
    CALUMPANG;

    3. That my wife and my children will be travelling to KOREA with me for a vacation from
    December 18-23, 2019;

    4. That I have sufficient income/ assets to responsibly cover my wife and children’s
    expenses during our entire trip to KOREA, including roundtrip airfare tickets from the
    Philippines to KOREA, hotel accommodations, food expenses, transport fares within
    KOREA allowances, pocket money and other unforeseen expenses which my family
    may occur during our entire stay in KOREA;

    5. As a husband and their parent, I hereby accept and assume any and all responsibility for
    their welfare, including all financial expenses related to their travel and stay in KOREA;

    6. I guarantee that we will abide by all laws and regulations of the KOREAN government;

    7. That I have executed this Affidavit of Support to attest of the foregoing fact of my
    capacity and capability of supporting my wife and children during their stay in KOREA;

    8. That this affidavit is executed in compliance with the requirement of their visa
    application;

    IN WITNESS WHEREOF, I have hereunto set my hand this 17th day of September
    2019 at Dumaguete City, Negros Oriental, Philippines.
