    RF: ORDER THE CANCELLATION AND REFUND OF UNAUTHORISED DEDUCTION FROM PAYSLIP
    Refer to the above caption matter.
    I DAVIES MASUMBA do hereby order the company to terminate the unauthorised deduction of the money
    amounting to four hundred kwacha only (K400.00) bearing the policy name “Zam State Ins Swift Sav” which was
    infected in February 2019. Going forward also, the company is being ordered to refund the total deduction of one
    thousand four hundred kwacha only (K1400.00) that is the money deducted from February 2019 to June 2019.
    The company and I are aware that any intrusion in terms of unauthorised deductions amounts to matters of theft
    and prosecution by relevant authority. Therefore before further actions and relevant authorities are involved, do
    kindly refund the deducted money into the cavmont account number : 200000342469, account name : DAVIES
    MASUMBA.
    Find attached are the copies of my NRC and latest payslip.

    Your wise and timely response to this letter shall be of great help to both parts.

    RF: CAUTION FOR FAILURE TO remit THE ASSURED SOME OF POLICY NO# C00065764
    I DAVIES MASUMBA do write a letter of caution to the company for failure to remit the sum of seven thousand
    five hundred kwacha only (K 7, 500.00) even after forwarding the claim form to the company from December
    2018.
    Therefore, this letter saves as a last warning to the company that failure to remit the stated amount will invoke
    the involvement of stakeholders and relevant authorities in the matter.
    Hence find the attached photocopied police document, NRC, claimant form and the last deduction of the same
    police. Please do remember that typing, photocopying and printing of the documents costs money, so failure to
    exercise civility in this matter shall not be condoned any more.

    Your response to this matter shall be of great help to both parts.
