    AFFIDAVIT
    (Change Color of Motor Vehicle)

    I, ATTY. HECTOR J. DIAZ, of legal age, single, Filipino and a resident of


    Magdalo St., LaPaz, Iloilo City, under oath, hereby depose and state, THAT:
    1. I, ATTY. HECTOR J. DIAZ, am the bonafide buyer of a motor vehicle
    registered under PCI(BDO) Leasing & Finance, Inc., who sold said motor vehicle to
    Monsanto Philippines, Inc., and who in turn sold the same to their employee, Josel A.
    Demetria, and who likewise sold the same motor vehicle to me, under a Deed of Sale
    Motor Vehicle of which motor vehicle I am now the true and absolute owner, being more
    particularly described as follows:

    2. The original color of the above-described motor vehicle was “SEL/TITANIUM


    GRAY MICA”;

    3. When the paint of the above-described motor vehicle had faded, I had it
    repainted and took charge of repainting it from the original color “SEL/TITANIUM
    GRAY MICA” to “RED” such that the color of the said motor vehicle is now changed
    from “SEL/TITANIUM GRAY MICA” to the current color “RED”;

    4. The said repainting/change of color is not intended for any illegal or unlawful
    purpose but purely for aesthetic and preservation purposes only; and

    5. That I am executing this Affidavit to attest to the truth of the foregoing and in
    support of the application for the change of color of the above-described motor vehicle on
    its Certificate of Registration.
