Why To Implement Safety Rules?
 Number of incidents and severity are increasing;

 We recognized the need to identify the significant risks in our areas of operations;

 Historical data reveals that 83% of threats associated with fatalities were related to
human factors;

 Rule-based errors were increasing;

 Need to focus on behavior safety;

 The requirement of simple, clear and direct communication to staffs especially to


supervisors and managers to understand these hazards that are most likely to
contribute to fatal incidents.
How To Implement Safety Rules?
 Comparative study on the ‘Safety Rules’ of organizations from Oil & Gas Producers
(OGP) – Benchmarking:

 Utilizing the statistics and resources from the workshop conducted by Oil and Gas
Producers association (OGP) – ‘Fatality review workshop’ in January 2010;
 OGP recommended 8 core rules and a library of additional 10 rules for member companies to chose from;
8 core rules apply to at least 70% of OGP reported fatal accidents;
 Advised the member companies to adopt the 8 core rules as a minimum.

 Study on the causes and contribution factors related to the Fatalities in the similar
industries = 90% prevention.

Safety Rules Requirements
 Simple, standardized terminologies;
 Pictorial representations – Understandable by workers from all geographical locations,
irrespective of their literacy;
 Focus on workplace hazards that most likely may result in fatality;
 Clarity and fairness.

Appropriate PPE shall be worn in the workplace as required by Orpic procedure &
displayed sign.

 The Minimum PPE to be worn at worksite:

 Hard Hat - Helmet

 Safety Glasses / Goggles

 Ear Protection (where required)

 Hand Gloves

 Long sleeve coverall or long sleeve Jacket 100% cotton

 Safety Boot / Shoes

Working or walking immediately under / close to a suspended load is unsafe as the
load can fall on you.

 No one should ever be under / close to any working crane or suspended load;

 Mark the unsafe area and put barriers in place;

 Never cross a barrier controlling an area with a suspended load without authorization;

 Follow the instructions of the Person In Charge of the lift;

 Trained Banks-men must be present at all times during work;

 Security of the load to be checked and tag lines attached prior to the lifting;

 Every lift must have a lifting plan refer to Crane entry certificate.

 Only certified trained personnel with valid training status, can issue & receive
a permit to work;

 Obtain a valid Permit to Work (PTW) whenever required for the task at hand;

 Work requiring PTW will not starts until the permit had been signed, issued
and communicated.

 Do not enter confined space or start hot works before gas test completed;

 Gas tests shall be carried out by Qualified gas testers who has completed Gas
Detection and Monitoring Training and certified by Orpic Training Department;

 Follow PTW instructions / Job Safety Analysis (JSA) for further gas tests.

 Permit to Work (PTW) must always be obtained before starting work in electrical &
mechanical isolation;

 Must communicate hazards identified in PTW and Job Safety Analysis (JSA) for each
step of job during the TBT (Toolbox Talk);

 Verify & Confirm isolation is in place, for example lock switches, separate pipes with
blinds;

 Verify & Confirm no stored energy or other dangers remain.

Confined space is an area that meets all of the following criteria:
 Has limited or restricted means for entry or exit;

 Is not designed for continuous human occupancy;

 Examples:

 Tanks  Vessel
 Manholes  Pipes
 Boilers  Trenches
 Furnaces  Pits
 Sewers  Excavation
 Vessel (1.2 m)

 Hazard identification and JSA must be completed


before entering confined space;

 Persons who may be required to enter confined


spaces or work as a standby person must have
completed a specific training in confined space
entry and be certified by Orpic Training
Department.

 Override is temporary bypass of a safeguarding systems, to allow certain work to
proceed without causing an unnecessary shutdown or fire alarm, e.g. fire detector
overridden during welding;

 This must be strictly controlled.

 For Safety System Overrides Ensure:


 Risk Assessed and authorized by the correct level of authority;

 Recorded in the override Log when applied and removed;

 Are managed and removed when job is completed;

 Are only applied by competent personnel;

 Are handed over to the next shift if left in place during a shift change;

 Are inspected and the status verified by supervision at a regular

 It is FORBIDDEN to carried matches and lighters into plant / process areas or areas
where there is potential for explosive or flammable gases, liquids or other materials;

 Smoking is only allowed at designated smoking areas, as indicated with a sign;

 Smoking materials must be discarded into ashtrays or other designated receptacles to


reduce the chance of fire.

 Do not over speed, Maximum speed limit in Orpic is 30 km/h;
 Don’t use the mobile phone while driving, if you really must answer the phone just pull
out your car and stop at the side of the road;
 Do not overtake the vehicles once your enter Orpic premises.

 Never park your vehicle near fire emergency
