    Blended Learning – PHD EM

    Requirements for portfolio:

    A4 Paper

    Instruction:

    a. Use the adapted PDF I sent you on FB messenger via Mr. Jemson.
    b. Disseminate the survey to at least 10 High School or College teachers that use at
    least 1 of the technology tools (projector,multi media, laptops, tablets,etc..). Let
    them answer as honestly as they can.
    c. Tally the results.
    d. Tabulate (put in tables each results of the sections)
    e. On a separate A4 paper, write an introduction of who you surveyed, where are
    they from, which school, what do they teach and what kind of technology they use
    in their classrooms.
    f. On each table, interpret the results.
    g. On another A4 paper, discuss the results & findings of the survey. (On another A4
    paper, discuss your own Analysis and Discussion about the results & Findings. you
    can use the readings as guide to help you analyze results of your survey)
    h. Collect all the raw data and staple them as one to insert in the portfolio.

    4. Create a Concept Proposal integrating technology for your “Future School” by


    answering the questions:
    (Download any model/template of a concept proposal and follow the format or you
    can use my template)
    a. If you were to be an administrator of a school, what kind of technology will you
    integrate and use in the classrooms?
    b. How will you prepare your teachers to adapt digital literacy?
    c. What are the different kind of technology will you be using in the classrooms?
    Describe each kind of technology and explain in detail how will it be used and
    for what purpose.
    d. How will you evaluate the effectiveness of each kind of technology used in the
    classroom? Give concrete examples.
