3 Identify the environmental aspect of each activity with the following


categories:
1. Discharge to Air: dust, gas (smoke, vapor, fume), etc.
2. Discharge to Water: oil spill, chemical waste spill, etc.
Likelihood Value

4. Usage of Raw Material / NaturalResources: water, paper, laboratory
samples, etc.
5. Usage of Energy: electric, etc.

forms.

Consequence Value

4 Identify any hazards caused by the work activities with the following
categories:
1. Unsafe Action;
1.a. Physiologic / Ergonomic: manual handling, repetitive movement,
working position, etc.
1.b. Psychological: working over-time, intimidation, violence, fatigue etc.
1.c. Behavioral: not wearing PPE properly, lack of concentration, short
cuts, violating procedures, etc.
2. Unsafe Condition:
2.a. Physical: height, slippery surface, surface with different elevation,
falling materials, confined space, electric shock, getting hit by moving
equipments, exposure to fire, explosion, radiation, noise, vibration,
extreme temperature, etc.
2.b. Chemical: contact with chemical substances, inhalation, ingestion,
unproper storage, etc.
2.c. Biological: virus, bacteria, snake bite, stung by insects, food
poisoning, food contamination, etc.
