MINUTES OF THE MEETING ON COLLABORATIVE PERFORMANCE TASK

Agenda :

I. The meeting started at two thirty in the afternoon. All Faculty members, Administrative Head and
School Directress were present.
II. An Opening Prayer was led by Ms. Mary Kanashi Manzano.
III. There was a reading of the previous minutes of the meeting on the revisions of the Handbook. It
was emphasized that changes are made in age requirement for each level; computation of grades
and selections of awardees; thorough discussion on the disciplinary sanctions on the use of
gadgets; suspension of classes complying with DepEd requirements. Parents are not allowed to go
directly in the classroom became part of the standard operating procedures. Further revisions of the
handbook are going on.
IV. Main agenda is the discussion of the collaboration on the performance task per subject area/ per
level for the first quarter. Guidelines and procedures were restated for a better understanding and
for making meaningful and purposive performance tasks. (See attachment on the Performance
Task and Guidelines)
V. Subject teacher in every grade level talked about the alignment of their topics and learning
competencies and came up with performance task.
VI. Here is the list of the collaborative performance task.

Grade 1 The students are tasked to:
1. For Computer and Mathematics: Use charts and drawing in solving
mathematical problems
2. Araling Panlipunan and Health : Gumawa ng mga tungkuling dapat
gawin mga namumuno sa ating pamayanan
3. Science and Physical Education: Write tips on how to care for the

5. ESP, English, and Filipino: Write an essay/ reflection Paper
Grade 2 The students are tasked to:
1. Arts, Science, Physical Education: Poster Making
2. English and Filipino: Poem Recitation
3. ESP and Araling Panlipunan: Role Playing
Grade 3 The students are tasked to:
1. Science, Arts, and Health : Clay
2. Mathematics, Music, English: Math Dance Challenge
Grade 4 The students are tasked to:
1. Araling Panlipunan and ESP : Do a reporting/ conduct a News
Casting on how safety procedures before/during/after a calamity
strikes
2. T.L.E. and Computer: Make a documentation on significance of
ornamental plants in the environment
3. English, Filipino, and Physical Education: Write an essay/ reflection
on significance of Palarong Pinoy in Health and Wellness
4. Araling Panlipunan, Arts and Physical Education: Draw a symbol for
the significance of Palarong Pinoy in promoting healthy life style and
valuing the Filipino culture and tradition.
Grade 5 The students are tasked to:
1. Arts and Physical Education: Draw a symbol for the significance of
Palarong Pinoy in promoting healthy life style.
2. Araling Panlipunan, Arts, ESP, English, Filipino, T.L.E. and Science:
Cooking healthy food, Putting up exhibits and making documentation
for Nutrition Month with the theme: “Kumain ng wasto at maging
aktibo… push natin ‘to.”
Grade 6 The students are tasked to:
1. Araling Panlipunan, Arts, ESP, Filipino, T.L.E. and Science: Cooking
healthy food, Putting up exhibits and making documentation for
Nutrition Month with the theme: “Kumain ng wasto at maging
aktibo… push natin ‘to.”
2. English, Health, Science, Mathematics, and T.L.E.: Problem solving
using pizza for mathematics. For T.L.E, students will do the baking
of pizza while in Health, students make their own healthy recipe for
the pizza. In English, writing of the procedures and documentations.
Grade 7 The students are tasked to:
1. Music and Physical Education: Writing and interpretation of a song/
jingle
2. ESP and English : Responsibility Challenge and writing an essay
3. Araling Panlipunan, Filipino, and T.L.E: Baking Activity
Grade 8 The students are tasked to:
1. English, Filipino, Music, T.L.E: Making food vlogging
2. Araling Panlipunan, Arts, ESP, Filipino, T.L.E. and Science: Cooking
healthy food, Putting up exhibits and making documentation for
Nutrition Month with the theme: “Kumain ng wasto at maging
aktibo… push natin ‘to.”

Grade 9 The students are tasked to:


1. Music, Arts, Physical Education, Health, and Science: Cooking
healthy food, Putting up exhibits and making documentation for
Nutrition Month with the theme: “Kumain ng wasto at maging
aktibo… push natin ‘to.”
2. Mathematics and Computer: Making a database
Grade 10 The students are tasked to:
1. Music, Arts, Physical Education, Health, and Science: Cooking
healthy food, Putting up exhibits and making documentation for
Nutrition Month with the theme: “Kumain ng wasto at maging
aktibo… push natin ‘to.”
2. Araling Panlipunan, Computer , Science: Reporting of the safety
measures on before/during/after calamities and making signage

VII. Rubrics and instruction must be discussed and given to the students for the basis of appropriate
grading and scoring.
VIII. Teachers were advised for the documentation of the performance task.
IX. The teachers –in – charge of the Nutrition Month Celebration discussed the general reminders and
programs.
X. Action Plan:
1. A memo will be given to the subject teachers on the guidelines and procedures of the
performance tasks.
2. Each teacher must document the making of performance tasks and keep the best outputs for
exhibits. Best outputs must be collected and placed in their designated area or laboratory.
XI. The meeting was adjourned at exactly 4:00 p.m.
