    ISSUE / CAUSE

    According to World Nuclear Association, the Chernobyl explosion in April 1986 was the result of
    the flawed reactor design and it was operated with inadequately trained personnel. The Chernobyl
    plant using four Soviet-designed of nuclear reactor (RBMK-1000 design) which are recognized as
    flawed. The serious mistakes made by nuclear plant operators are also one of the main cause of
    the Chernobyl explosion.

    The accidents is happen on April 25, the day before the accident and prior to a routine shutdown,
    the reactor crew at Chernobyl 4 do a test to determine the time of turbines would spin and supply
    power to the main circulating pumps in order to loss electrical power supply. The result from the
    test shown that the power from the turbine ran down too rapidly.

    A series of operator action taken by nuclear plant operators are improper and inaccurate. They
    disabling the automatic shutdown mechanisms, preceded the attempted test on early April 26. This
    cause when the time operator moved to shut down the reactor, the reactor already under unstable
    condition. All of these improper action and the flawed of the rector cause the reactor overpressure.
    The overpressure caused the 1000 t cover plate of the reactor to become partially detached,
    rupturing the fuel channels and jamming all the control rods. Intense steam generation spread
    throughout the whole core causing a steam explosion and releasing fission products into the
    atmosphere. Second explosion occur after about two or three seconds due to the production of
    hydrogen from zirconium-steam reactions.

    Anatoly Stepanovich Dyatlov was the supervisor of the catastrophic safety test which cause the
    Chernobyl disaster. Dyatlov was overseeing a test of the nuclear power plant’s emergency safety
    mechanism and mishandling the event and fail to follow safety protocols. Chief Chernobyl
    engineer Nikolai Fomin, plant manager Viktor Bryukhanov and Dyatlov were convicted their
    alleged crimes and sentenced to 10 years in prison.
