INVITATION TO APPLY FOR CONSIGNMENT

Pursuant to Provincial Ordinance No. 007-2019, the Provincial Government of


Zamboanga del Sur, through the Zamboanga del Sur Medical Center (ZdSMC),
will be implementing a consignment system purposely to provide accessible and
readily available drugs and medicines.

Anent, the Bids and Awards Committee (BAC), invites suppliers, dealers and
distributors registered with the Bureau of Food and Drugs to submit the
following requirements as cited under Section 7 of the said ordinance and
eligibility documents provided by the implementing rules and regulations of
Republic Act 9184 or otherwise known as the Government Procurement Reform
Act of 2002, to wit:

1. Certificate from the Bureau of Internal Revenue acknowledging receipt


of duly audited Financial Statements;

2. SEC Registration Certificate for corporations/partnerships or relevant


DTI registration certificate for sole proprietorship;

3. Business Permit from the LGU which administratively covers the


consignee with which the consignor intends to enter into a drug
consignment agreement;

4. Sworn and duly notarized statement that the prospective consignor has
not been blacklisted to participate in biddings by any Government
Agency, LGU or Government Owned or Controlled Corporation (GOCC);

5. List of Drugs and Medicines that are intended to be consigned together


with the corresponding certified true copies of Certificates of Product
Registration issued by the BFAD and

6. Certificate of Current Good Manufacturing Practice (CGMP) issued by


BFAD (for manufacturers). In case the prospective consignor is not a
manufacturer, its principal manufacturer’s certified true copy of CMGP
certificate must still be submitted.

7. Tax Clearance per E.O. 398, series of 2005, as finally reviewed and
approved by the Bureau of Internal Revenue (BIR)

8. Statement of the prospective consignor of all its ongoing government and


private contracts, including contracts not yet started, if any, whether
similar or not similar in nature and complexity to the contract of
consignment

9. Statement of the consignor’s Single Largest Completed Contract (SLCC)


similar to the contract to be bid, except under conditions provided for in
Section 23.4.1.3 and 23.4.1.4 of the IRR of RA 9184, within the relevant
period as provided in the Bidding Documents in case of Goods.

All of the above statements shall include all information required in the
PBDs prescribed by the GPBB

10. The prospective consignor’s computation of Net Financial Contracting


Capacity (NFCC). However, in the case of procurement of Goods, a
bidder may submit a committed line of credit from a universal or
commercial bank, in lieu or its NFCC computation.

Prospective Consignors shall submit the following Eligibility Requirement to the


BAC Secretariat Office, Second Floor, Provincial Capitol Bldg., Pagadian

for the eligibility screening. The results of the Eligibility Check may also be
obtained in the Office of the BAC Secretariat.

The Provincial Government of Zamboanga del Sur assumes no responsibility


whatsoever to compensate or indemnify prospective consignors for any expenses
incurred in the preparation of their documents.

Furthermore, the Provincial Government of Zamboanga del Sur reserves the right
to reject any of all consignment offers, to annul the consignment process, and to
reject all consignment any time prior to contract award without thereby incurring
any liability to the affected consignor or consignors.
