    AFFIDAVIT OF LOSS
    I, MIRIAM PARATO FLORES, of legal age, Filipino, widow, with residence and
    postal address at Block 65 Lot 2 ACM Woodstock, City of General Trias, Cavite, after
    having been duly sworn to in accordance with law, hereby depose and state:

    1. That I am the wife of the late RUBEN DANTE BATAC FLORES, the latter being a
    bonafide Planholder of ST. PETER LIFE PLAN, INC. under Life Plan Contract
    No. L18015574C, Memorial Service St. Gregory;

    2. That the original documents issued in favor of my deceased husband, RUBEN


    DANTE BATAC FLORES by the ST. PETER LIFE PLAN, INC. pertaining to the
    said Contract Policy has been misplaced and could not be found;

    3. That I exerted all efforts to look for the said Contract Policy but to no avail;

    4. That I executed this affidavit to attest to the truth of the foregoing facts and
    circumstances and to establish such loss for the purpose of securing a new copy
    thereof from ST. PETER LIFE PLAN, INC.;

    IN WITNESS WHEREOF, I have hereunto set my hand this 11th day of October,
    2019 at the City of General Trias, Cavite, Philippines.
