    report on pendency and non-pendency of any administrative
    and disbarment case

    Dear Sir or Madam:

    Peace greetings!

    I would like to request from your good office to issue in my favor, a certificate of
    good standing and report on pendency and non-pendency of any administrative
    and disbarment case.

    This certificate/s will be used in order to comply with the requirements for the
    application of Notarial Commission in my Province of Negros Oriental in which I
    will be filing a petition for, when all the needed requirements are complied with.

    Herein attached is a machine copy of my Certificate of Membership in the


    Philippine Bar bearing Roll No. 70587 and IBP Receipt No. 043305.

    Also, together with my letter is BDO Receipt showing proof of payment of the
    amount of P200.00 for the request of the certification/s.
