    PRIVATE SCHOOLS ASSOCIATION OF PARANAQUE (PSAP)

    I. ELIGIBILITY OF ATHLETICS

    Based on DEPEd Memoramdum (July 20, 2015)


    Rules on Participants

    1. A Filipino Citizen
    2. The athlete must be a bonafide student of the school he/she repre-
    sents and must be officially enrolled of the current school year.
    3. For elementary athlete, he or she should been born in 2007 or later;
    And for secondary athletes, he or she should have been born in 2002
    or later. Thereafter, add one (1) year to the cut-off date for the suc-
    ceeding year.
    4. As much as possible No failing grades, however if there is a failing
    grade (no more than 2 failing grades is acceptable).
    5. As a general rule, elementary athletes shall play in the elementary di-
    vision and secondary athletes shall play in the secondary division.
    However, elementary athletes who have been declared overage in the
    elementary level may be allowed to play in the secondary level.

    II. CREDENTIALS AND DOCUMENTS REQUIRED (STUDENTS)

    All papers must be printer in A4 bond paper size.

    1. AR-1 Form
    Note: For the AR-1 please put the Place of birth, not the Hospi-
    tal.
    2. Four (4) pcs. Picture 1.5” x 1.5” in white background
    Format: Surname, First Name, Middle Initial
    Grade and Section
    3. Original Birth Certificate (PSA) (No PSA Birth Certificate, No
    Play)
    4. Certificate of Enrolment with the Principal’s signature
    5. Medical Certificate (3 copies for Combative Sports)
    6. Parental consent in the absence of parent, please submit an
    affidavit of legal guardianship. (attach a clear photocopy of gov-
    ernment issued ID with signature)
    7. For OLD Athlete student with Form 137, duly signed by
    the registrar and principal (EXEMPTION!!! For newly trans-
    ferred student/athlete of the school, please submit form 138 or
    report card in lieu of form 137 from the previous school together
    with the letter from the principal certifying the athlete is a new
    student to their institution.)

    III. CREDENTIALS AND DOCUMENTS REQUIRED (COACH)


    - Certificate of Employment (If a parent, provide school’s
    acknowledgement letter regarding parent coach)
    - Picture 1.5” x 1.5” in white background with Name (same format
    as that of the students)
    - Personal Data sheet
    - Medical Certificate (For City Meet Coaches only)
    - PSAP ID (PSAP will provide)

    SCHOOL REQUIREMENT:
    1. All schools are required to fill up the template and be sent
    together with the gallery on screening dates.

    Note: Please refer to e sports form generator. All the data must be
    entered only in the INPUT DATA sheet. All forms must be in A4 size
    and computerized. Handwritten documents WILL NOT be screened.

    After the PSAP Meet the respective school will be held responsible in
    retrieving their documents.
