ADVICE TO DIABETIC PATIENTS DURING ILLNESS
Dr.Varesh Nagrath ,M.D. SICK DAY PROTOCOL Things which you must have preferably at home Glucometer Ketostix :Which measures urinary Ketones. Do frequent Blood Sugar tests at home with your Glucometer ,as frequent as every four hours.If you do not have glucometer inform your Doctor/lab for help. Get urinary ketons tested 4 hourly (Very important if you have Insulin dependent Diabetes ) once a day (if you have Type 2 diabetes) ,or Lifestyle Modification Take full rest.do not exert . Diet modification Small frequent meals are advised. Drink clear liquids frequently ( for eg ½ cup of Clear soup every hour) Eat light food if you can. For eg Toast /Juice/ Diet Coke /Namkeen of moong dal.. Recall the light snacks you relish and consume them. Take candies Drug modification This should preferably be done by your Doctor. This depends upon the level of blood glucose & associated symptoms. Blood glucose might be high (due to insulin resistance ) or low (because of low appetite & intake) during sick days. If you take insulin : Your appetitie might be less during sickness,however,do not miss insulin.In fact,the requirement of insulin might increase during sickness as it becomes more difficult for insulin to be effective.
If you take Drugs belonging to Sulfonylurea group
Do not increas the drug. Any increase in Blood sugar values is to be controlled by Insulin. Decrease the dose if the blood glucose is falling low. If you take Metformin Stop metformin Start insulin for the period of sickness. Adjust dose according to blood values. You need to show to your Doctor if Illness lasts > 2 days Vomiting ,Diarrhoes. Blood glucose value > 400 mg % Ketones appear in the urine. You feel very ill or there is an obvious infection You feel breathless ,fatigued ,giddy. Guidelines for Doctors for Adjustment of Insulin Dose during sick days ( Continue the previous treatment. Do 3-4 hour RBG & Ketonuria testing) If there is no ketonuria and RBG is 90-160 mg % -Do not add insulin. RBG is 180-250 mg % -Add 2 units of fast acting insulin RBG is 250-350 mg % -Add 4 units of fast acting insulin in that dose. RBG is 350-400 mg % -Add 6 units of Regular insulin
