o. CSC Form 212 (Revised 2005) in two copies wiih lhe lotest 2x2tD piciure
b. Certified photocopy of PRC professionol identificotion cord or cr pRC
certificotion showing the teocher's nome, LET roting, ond other informotron
recorded in the PRC Office (COPY MUST BE AUTHENTTCATED By pRC)
c. Certified photocopy ol rotings obloined in the Licensure Exominotion for
Teochers (LET)/Professionol Boord Exominotion for leochers (PBET) (COPY MUSI
BE AUTHENTICATED BY PRC)
d. Certified copy of Tronscript of Records (COPY MUST BE AUTHENTICATED BY IHE
SCHOOL WHERE THE APPLICANT GRADUATED FROM WITH CERTIFIED GENERAL
WEIGHTED AVERAGE)
e. Copies of service records, performonce roling, ond school cleoronce for
those with teoching experience. lf unovoiloble, the opplicont must submil o
justificotion citing the reosonls for unovoilobility, For teochers who hod served
os locolly-poid teocher of lhe Locol School Boord (LSB), their experience/s os
