    PROJECT NAME : REPAIR / CONSTRUCTION OF ANGANCASILIAN BHS

    CONTRACTOR : FOREF CONSTRUCTIOM

    This is to certify that FOREF CONSTRUCTION, ILAGAN, ISABELA, under contract with
    DEPARTMENT OF HEALTH – REGION 2 in the amount of PHP 995,418.97 has been completed on
    OCTOBER 25, 2018

    The DOH Inspectorate Team has inspected the project on OCTOBER 25, 2018 together with the
    representative of the Contractor. Attached is the copy of the Inspection Report. The project was re-
    inspected on ______N/A________ by the Inspectorate Team if the noted defects/deficiencies have
    already been corrected/rectified.

    Since the contractor has satisfactorily complied with the requirements of the Inspectorate Team,
    this Certificate of Completion is hereby issued.

    This certification, however, does not relieve the Contractor of his obligations and responsibilities
    in undertaking repair works, in accordance with the accepted standards, on any defects or failure that
    may occur on any section of the project during the one (1) year Defects Liability Period. During this period,
    the contractor is responsible for the repair on any defects, fail wear and tear expected, and these should
    be rectified by the contractor at his own expense prior to final acceptance.

    The one (1) year Warranty Period commences from the date of final acceptance.
