Indications:
1. Symptoms compatible with acute myocardial infarction
2. Time to treatment of 6 hours to 12 hours
3. EKG with at least 1 mm ST segment elevations in at least two contiguous leads
4. Interventional management is not available
Contraindications to therapy:
Absolute
1. Previous hemorrhagic stroke at anytime
2. Cardiovascular accident (CVA or stroke) in the past year
3. Known intracranial neoplasm
4. Active internal bleeding (exclude menses)
5. Suspected aortic dissection
6. Diabetic retinopathy
Relative
1. Significantly elevated blood pressure (>180/100 mm Hg)
2. History of prior CVA
3. Current use of anticoagulants (with an INR >2-3)
4. Trauma within the past 2 weeks
5. Known bleeding diathesis
6. Prolonged cardiopulmonary resuscitation (>10 min)
7. Major surgery in past 3 weeks
8. Noncompressible vascular punctures
9. Recent internal bleeding (2 to 4 weeks)
10. Prior allergic reaction to fibrinolytic
11. Pregnancy
12. Active peptic ulcer disease
13. History of chronic severe hypertension
