Purpose
5-Why Analysis Worksheet is a tool for finding specific causes for a specific problem, as it breaks down the problem to its root cause. I
be used when the basic cause(s) to a problem should be found, and for preventing problems that have not yet occurred. Using this too
give you reassurance that you are implementing corrective actions to a problem by addressing the root cause and not just the sympto

Facilitating Tips
The template "Why Analysis" has 5 times Why, but you might experience that you find the root cause after 2 or 3 Whys, or you might
need more Whys. If you can continue to dig deeper into the root cause then you should continue doing so until you find the root cau
Be careful that the answers you get to the root cause do not end up being too broad.
This tool can be used together with the Fishbone Diagram where the causes that are identified in the Fishbone Diagram are further
investigated by using Triple -5 Why Analysis Worksheet.

give you reassurance that you are implementing corrective actions to a problem by addressing the root cause and not just the symptom of
the problem.
Instructions
Describe the problem as specifically as possible.

Make an analysis for each human performance problem. You need to add a line or a sheet for each problem.
Ask up to 5 times, Why? Do not accept the root cause, until you get into depth with it and find the actual root cause.
In the right side of the document, write which corrective actions that has been implemented to correct the problem.
Make an analysis for each equipment problem. You need to add a line or a sheet for each problem.
Ask up to 5 times, Why? Do not accept the root cause, until you get into depth with it and find the actual root cause.
In the right side of the document, write which corrective actions that has been implemented to correct the problem.
Finally, consider what the lessons learned are and write these at the bottom of the sheet.
