    AFFIDAVIT OF LEGITIMATION BY SUBSEQUENT MARRIAGE

    WE, RAMIL G. ESPIGOL and JUVY DE LA SERNA-


    ESPIGOL, of legal age, Filipino, and with postal address at 1-7-B
    Beatris St., L&B II Ilaya Compound, Alabang Muntinlupa City,
    after having been sworn in accordance with law, hereby depose
    and say that:

    1. That we are the parent of JOMMEL DE LA SERNA who


    was born on March 21, 2000, in Muntinlupa City, copy of
    the Birth Certificate is here to attached as Annex A and
    shall form an integral part hereof;

    2. Our son JOMMEL DE LA SERNA, was born out of wedlock;

    3. When our son was conceived and born, we both possessed


    no legal impediment to marry each other;

    4. On August 23, 2000, we got married to each other and our


    marriage was solemnized at Office of the Municipal Mayor ,
    Barcelona Sorsogon, copy of the Marriage Contract is
    hereto attached as Annex B and shall form an integral
    part hereof;

    5. We executing this Affidavit to attest to the foregoing facts,


    for purposes of complying with the requirements in
    applying for legitimation by subsequent marriage of our
    son, and for the proper recording of such legitimation in his
    civil registry document, to enable him to bear surname of
    his father and be entitled to all the rights of legitimated
    child, and for all legal intents and purposes.

    IN WITNESS WHEREOF, I have hereunto set my hand this


    12th day of December 2016, in the City of Paraaque.
