DRUG FORMULARY
Foreword
With the exponential advancement in the medical technologies, there is a
challenging need for all healthcare professional to equip themselves with
adequate and updated drug information in order to deliver safe, appropriate and
cost-effective healthcare services to patients.
In conjunction with this, Hospital Pakar Sultanah Fatimah has published its own
Hospital Drugs Formulary with the purpose of achieving the objective above as a
handy reference and to encourage an effective and efficient prescribing practice
in Hospital Pakar Sultanah Fatimah.
The category of prescribers that are authorized to initiate the prescription for the
drug in this formulary is listed as:

I hope that this Hospital Drugs Formulary will not only serve to improve the
standards of medication prescribing in Hospital Pakar Sultanah Fatimah but also
as an educational reference for new doctors and pharmacists.
Lastly, I would like to express my appreciation to the Drug Information Unit of the
Pharmacy Department HPSF for their time and continuing efforts undertaken in
producing this Hospital Drug Formulary.
Foreword
The Ministry of Health (MOH) Drug Formulary contains a list of drugs approved
for use in all hospitals or institutions under the Ministry. The Formulary is to
promote rational, cost effective use of drugs whereby newer and more expensive
drugs were introduced in a controlled manner so as to minimize wastage. The
use of the Formulary promotes optimal utilization of funds to purchase drugs.
This Hospital Drug Formulary is adapted from the latest MOH Drug Formulary.
With the large number of new drugs and formulations listed in the Drug
Formulary of the Health Ministry, it is appropriate and timely that guidance is
provided to assist doctors in Hospital Pakar Sultanah Fatimah, Muar in selecting
drugs and formulation that is available in this hospital. A review and update of
this hospital formulary will be done periodically to keep up with the changes in
the availability of drugs, treatment guideline and feedback from the clinical head
of departments. For this Formulary to be successfull, it must be acceptable in our
hospital and used wisely among hospital staff. We welcome feedback from all
hospital staff to improve the quality of this Hospital Drug Formulary.
This Formulary depends heavily on contribution from the Drug Information Unit of
the Pharmacy Department, who generously spend their time and effort to collate
a brief but important document, for the benefit of this hospital. I congratulate
them for their effort.

dosage may then be reduced to about

drug and continued for an additional 4 5 weeks.
CHILD: 1 mg/kg/day divided into 2
equal daily doses; after 2 to 4 days

and continued for approximately 1
month.
