1. 2. It is presumed that the specimen belongs to the patient named or identified in the Test Request Form. Test requested might not be performed for the following reasons: * Quantity of specimen received for the test is insufficient (QNS) (inadequate collection/spillage in transit). * The specimen received is of unacceptable quality ( heamolysed/clotted/lipemic) * Incorrect specimen type. In any of the above instances, a fresh specimen must be sent for reporting of the same parameter(s) if required. If the collection date was not stated in the Test Request Form, the same will not be printed on the report. Some tests are referred to other laboratories to provide a wider test menu to the patients. The details of the laboratory, where the sample was referred to, can be obtained from the Customer Care Services. Partial reproduction of this report is not permitted. Laboratory investigations should be used along with relevant clinical examinations to achieve final diagnosis. These are never conclusive and are dependent on the quality of the sample received by the laboratory as well as the assay procedures used.
