NURSING CARE PLAN Nursing Diagnosis # 1: impaired physical mobility r/t decreased lower muscle strength secondary to nervous

insufficiency as evidence by immobility of the lower extremity Goal: To promote physical mobility Expected Outcome: At the end of 8 hours, the patient will: a. Maintain or increase strength and function of affected or compensatory body part. b. Demonstrate techniques and behaviors that enable resumption of activities. Interventions Promotive 1. Assess functional ability and extent of impairment initially and on a regular basis. Preventive 2. Change positions at least every 2 hours. 3. Observe affected side for color, edema or other signs of compromised circulation. Curative 4. Begin active or passive ROM to all extremities. 5. Assist client to develop sitting balance. 5. Patient was able to sit up without discomfort. 4. Patient was able to perform ROM to all extremities. 2. Patient was turned every 2 hours and stayed comfortable. 3. There was no jaundice noticed, therefore there was good circulation 1. The patient was able to move right extremities without difficulty or discomfort. Response / Performance Indicator
