    THE CUSTOMER UNDERSTANDS THAT THE TERMS OF WHICH MAGIC VALLEY


    GRANTS CREDIT ARE:

    TERMS:

    Accounts are due and payable upon receipt of invoices, net 15 days.

    A finance charge of 1.75% per month (21% A.P.R.) will be charged on all past due
    accounts over 30 days.
    2.

    AGREEMENT:
    I (We), the undersigned hereby grant Magic Valley permission for
    a credit check on the above listed business and individual(s). In the event of default by
    the customer(s), in the performance of any of the terms and conditions hereof, Magic
    Valley may thereupon proceed to collect from the customer(s) the entire balance due, and
    if the matter be placed in the hands of attorney for suit or collection, the customer(s)
    agree to pay all reasonable attorneys fee for such action as well as any and all costs of
    collection.

    THE ABOVE TERMS AND CONDITIONS ARE HEREBY UNDERSTOOD AND AGREED

    PERSONAL GUARANTEE:
    The undersigned personally guarantees payment to
    Magic Valley on demand, of all indebtedness of the customer named above. This
    guarantee is without limit as to amount and may only be revoked by written notice to
    Magic Valley for transactions occurring after receipt of the notice. The undersigned
    waives any right to separate notice of delinquency or to require Magic Valley to proceed
    against the customer or any security, and further agrees to pay a reasonable attorney fees
    and other costs incurred by Magic Valley in the enforcement of this guarantee.
