LETTER OF APPEAL REQUESTING FOR RECONSIDERATION
OF LATE FILED CLAIM

FERNANDEZ Head, Claims Review Office Sir: The undersigned is the widowed wife of the deceased ANTONIO NICDAO DUCEPEC who died at St. Lukes Medical Center on November 23, 2002 and this refers to our appeal for reconsideration of our filed claim for PHILHEALTH benefits not credited to the hospitalization of my husband, which claim was not granted by reason of its delayed filing. Please allow us to avail of this Philhealth benefits at this time for it was only recently that we learned of the deadline for filing such benefits. This appeal is prompted by the following reasons: 1. My husband was confined at the Coronary Care Unit of St. Lukes Medical Center for three (3) weeks. Unfortunately, after several medical attention accorded to him for his recovery, complications occurred and he suffered from a cardiac arrest on the evening of November 23, 2002. This sudden and unexpected turn of events came as a surprise to all of us since the doctors were already preparing him for recovery and scheduled his release from confinement. 2. After the death of my husband, we were not advised by St. Lukes Medical Center of the sixty (60) day period to file and process all Philhealth documents, thus, we prioritized instead the processing of his SSS death benefit for we were badly in need of the funds immediately. Considering that it was the first hospital confinement of my husband since he became a member of the Medicare for almost 45 years until his death on November 23, 2002, we feel due consideration and understanding in our claim for whatever benefits due us, his bereaved family. May we reiterate therefore our appeal for your favorable consideration.
