    APPLICATION PROCESS

    1. Fill out the Online Shifting/Transfer Application Form at http://cba.upd.edu.ph/.


    2. Complete and print the Shifting/Transfer Application Form (this form). Make sure the information on your online
    application matches the information on your printed Application Form.
    3. Pay the P30.00 application fee at the Cashiers Office (UPD Account No. 9682-500-499-450).
    4. On or before 19 June 2015, submit (a) the printed Application Form (this form); (b) application fee Official Receipt; and (c)
    TCG/TOR issued by your college/unit (not required for VSB students shifting to/from BSBA/BSBAA).
    5. Wait for the results to be posted not later than 10 July 2015.
