    Fold at scored line and glue front to back.
    Write name and address on back by hand, and sign card.
    In box marked "CLASS" either enter the expected year of graduation of the bearer, or
    a letter "A" or "B." Class A library cards are for graduate students and teaching
    assistants. Class B cards are for undergraduates.
