    WILLOW CREEK COUNTRY CLUB, HEPPNER, OREGON


    PLEASE ATTACH YOUR $50 FEE PER GOLFER (If you are registering a team, please
    include the names of all players. We will do the best we can to accommodate but please
    return the forms ASAP.)

    Please be at the Willow Creek Country Club by 8:30 a.m. in order to get with your
    team.
    Golfing will begin at 9:00 a.m.
    Beverages and snacks will be available on the course.
    There will be a putting contest. It will run all day.
    Please contact the Willow Creek Country Club (541-676-5437) if you want to rent a
    golf cart. Limited carts available, please call ASAP.

    The $50 fee includes golf, lunch, and beverages. Creative Care Pre-School is providing
    lunch.

    Proceeds from the golf tournament will be donated to the Jason Halvorsen and Mike
    Matthews Scholarship Funds and 4

    of July Celebration Fund.
