    STATEMENT OF EXCLUSIVITY AND AVAILABILITY Project request: Development and implementation of a comprehensive training program for Paediatric Intensive

    Care Units Thematic Fund Reform Fund linked to Health Issues SWISS ROMANIAN COOPERATION PROGRAM 2013
    I, the undersigned, hereby declare that I agree to participate exclusively with the organisation Center for Health Policies and Services, Romania in the above-mentioned request for services for the implementation of the project Development and implementation of a comprehensive training program for Paediatric Intensive Care Units. I further declare that I am able and willing to participate in the project implementation for the position for which my CV has been included in the project proposal. The exact length and data of my consultancy during the 2 implementation years (2013-2015) will be agreed with the Center for Health Policies and Services ones the detailed project implementation plan will be discussed and agreed with the financing body. I confirm that I do not have a confirmed engagement as expert in another proposal under the same call or any other professional activity incompatible with the above engagements and that I shall avoid such conflicting assignments. Should any such conflict seem probable, I shall notify without delay the respective contractor. By making this declaration, I understand that I am not allowed to present myself with another organisation short listed for the above mentioned request for proposals for project implementation. I declare also that I am fully aware of my rights and obligations having read and understood the terms of reference for the above mentioned project.
